Clinic must be located within a 35-mile radius from the center of Ponca City, Oklahoma and be located within our primary service area in the State of Oklahoma. Physicians and technicians operating under the resulting contract must meet all evaluation criteria and other professional certifications required to operate in such a clinic. Clinic profiles will be established as clinic staff deems appropriate to accommodate workload demand. Solicitation issue date shall be on or about June 1, 1995.
