This action is to provide two (2) eight hour clinic days per week, for a 52 week period, of professional primary optometric care. This will include optometric examinations, vision screenings, specialty care, treating and monitoring ocular diseases. Services will be provided in the Dulce Indian Health Center in Dulce, New Mexico. All necessary Optometric equipment, supplies and forms, office space, utilities and local telephone service will be available. Clinic visits will be conducted exclusive of travel time and shall exclude federal holidays and weekends. A single award is expected to result from this solicitation. Solicitation will be issued on or about April 27, 1995, Period of performance to be effective one year from date of award and includes four (4) one year option periods. This is a 100% Small Business Set-Aside. No telephone requests will be accepted. All requests must be in writing. (102)
