F -- APPLICATIONS FOR INCLUSION ON RESPONSE ACTION CONTRACTOR LIST Notice is hereby given that the Louisiana Department of Environmental Quality, Underground Storage Tank Division will be accepting additional applications for inclusions on the established list of Response Action Contractors to conduct work at leaking underground motor fuel tank sites eligible for cost reimbursement pursuant to Louisiana's Motor Fuel Underground Tank Trust (R.S. 30:2195). After July 15, 1995, no additions will be made to this approved Response Action Contractors list until November 15, 1995. Only those contractors listed will be authorized to conduct response action activities at sites eligible for reimbursement from the Trust. If you are already a member of the established list, please disregard this notice. Services required of the approved response action contractors will include, but not be limited to the following: 1. precision tank tightness testing of underground storage tanks and lines for leakage, utilizing a method approved by the Department; 2. soil borings of up to 100 feet for soil sampling and/or groundwater monitoring purposes, to include the installation, development, and sampling/monitoring of groundwater monitoring wells; 3. physical removal, treatment, and/or proper disposal of soils contaminated with motor fuels; 4. removal of underground storage tanks; and 5. design and installation/implmentation of corrective action programs for the purpose of remediating contaminated soils and/or groundwater at leaking underground storage tank sites, including the installation, operation, and maintenance of recovery wells as well as the treatment of contaminated groundwater. In order to be listed as an approved Response Action Contractor for conducting eligible response actions, contractors must meet the following minimum requirements: 1. be licensed by the Louisiana Board of Contractors; (please send copy of license with application) 2. have at least $1,000,000 contractor's general liability insurance; (please send copy of insurance with application) 2. ensure that employees/subcontractors comply with applicable Occupational Safety and Health Administration (OSHA) training and certification requirements. Please send copy of certificates or documentation of certification with application; 4. have relevant experience in environmental investigation and/or clean-up at sites exhibiting subsurface contamination. Please send with the application a history of the jobs that have bgeen performed; 5. have a registered professional engineer or geoglogist on staff or on retainer. If on retainer, please send name of geologist or engineer with application; 6. be certified by equipment manufacturer to perform precision tank and line tightness tests or have access to a manufacturer certified precision tank tightness tester; and 7. be able to begin work at any site within 24 hours after authorization from the eligible Trust participant. Please send written statement with application that youe will comply. Any contractor who is interested in being included on the approved Response Action Contractors List maintained by the Department of Environmental Quality as heretofore described, must submit a written statement, on or before 4:00 p.m., July 15, 1995, showing that, on the date of submittal, said contractor meets the minimum requirements listed in Items 1-7 above to Raul M. Any contractor who in the opinion of the Department meets the above requirements will be listed as an approved Response Action Contractor for the period of this listing. The Department reserves the right to request additional information from any contractor responding to this notice necessary to clarify whether it meets these minimum standards. A submittal in response to this notice must address each of the minimum requirements listed herein in order for that submittal to be considered. Responding contractors who cannot meet any of the minimum standards prior to July 15, 1995, will not be considered as qualifying for the period of this listing. (095)
