NYCTA Procurement Dept., 130 Livingston Street, 6th Floor, Brooklyn,
NY. The factors that the Authority will consider in evaluating
proposals will include in relative order of importance, the following:
for PHYSICIANS: (1) Knowledge of overall requirements; (2) Experience
in relevant areas; (3) Experience and qualifications of assigned
personnel; (4) Cost analysis (labor, overhead and profit); (5) Adequacy
and completeness of the proposal with regard to the work and all
matters called for in the RFP; (6) Project manning including
subconsultants; (7) Ability to supply back-up coverage; (8) Special
licensing and certification requirements as per scope of work; (9)
Compliance with all requirements as represented in the RFP and the
terms and conditions of the proposed Agreement. For REGISTERED NURSES,
LABORATORY AND X-RAY TECHNICIANS AND MEDICAL ASSISTANTS: (1) Knowledge
of overall requirements; (2) Experience in relevant areas; (3)
Experience and qualifications of assigned personnel; (4) Cost analysis
(labor, overhead and profit); (5) Adequacy and completeness of the
proposal with regard to the work and all matters called for in the RFP;
(6) Project manning including subconsultants; (7) Ability to supply
back-up coverage; (8) Special licensing by NYS Education Department and
Worker's Compensation Board, as required during the performance of work
as per the Scope of Work; (9) Compliance with all requirements as
represented in the RFP and the terms and conditions of the proposed
Agreement. A pre-proposal Conference will be held on Wednesday March
28, 1995 at 2:00 P.M. in Conference Rooms 5 & 6, 6th Floor, at 130
Livingston Street (at Boerum Place), Brooklyn, New York. Prospective
proposers who desire to attend shall notify the Procurement Specialist
Dyanne Sampson, at (718) 694-4094, no later than noon on March 27,
1995. All inquiries must be received by the Authority, in writing, no
later than close of business, April 3, 1995. All proposals are due no
later than April 24, 1995. The Authority intends to select a proposer
or proposers with which to enter into negotiations by May 22, 1995.

130 Livingston Street, Brooklyn, New York 11201, 1st floor Bid
Reception desk between the hours of 9:00 A.M. - 5:00 P.M. Sealed
proposals will be received by the NYCTA at 130 Livingston Street,
Brooklyn, New York 11201 at the Bid Solicitation Counter until the
proposal opening time and date. Proposers will be fully responsible for
the delivery of their submissions in a timely manner. Reliance upon the
U.S. Mail or other carriers is at the proposer's risk. Late submissions
