As a result, a RFP will be issued requesting that all deaths to fire service members in Calendar Year 1994 be identified and the situations resulting from the fatalities be analyzed and reported. Anticipated period of performance is 12 months with two (2) one year options. All request for RFP package must be in writing and shall be received within 15 days after the date of publication of this synopsis. Telephone request will not honored.
