R -- EXCAVATION, LOADING, TRANSPORTATION AND DISPOSAL OF THE WAYNE INTERIM STORAGE SITE PILE, BUILDING AND SUBSURFACE UNDER THE FORMERLY UTILIZED SITESREMEDIAL ACTION PROGRAM (FUSRAP) SOL 14501-191-SC-499 Addendum. Bechtel National, Inc., under Dept. of Energy Contract No. DE-AC05-91-OR-21949, is seeking expressions of interest from licensed firms for the excavation, loading, transportation and disposal of the Wayne Interim Storage Site Pile, Building and Subsurface Material. The volume of material to be disposed of is in the range of 40,000 to 110,000 cubic yards over a five year time period. Because of the unique requirements of this work, interested firms should demonstrate the following prerequisite capabilities; (1) must possess required contractors licenses and submit evidence that the firm has successfully completed work of this nature within the past 18 months including disposal; (2) must currently possess or be able to obtain necessary financial resources and insurance commensurate with an estimated price that is based on the approximate volume of material. All interested firms are invited to submit information to show compliance with the prerequisites which, as a minimum should include: (a) name and location of the potential disposal facility and a brief description of the facility's construction, capacity and site accessibility including rail, cost estimate, and customer's and regulatory contracts' names and phone numbers; (b) a statement that the interested firm will allow periodic assessments, audits, and/or technical reviews of the firm's disposal facility. The audits/reviews may include, but not be limited to, interviews with the firm's personnel, laboratory visits and reviews of laboratory procedures, reviews of QA/QC records, and reviews of material and sample handling; (c) copies of required licenses or a written plan that clearly and reasonably indicates that the required licenses have been applied for and the dates when issuance is expected; (d) a statement specifying how the firm will handle any limitations or restrictions on disposal such as required state approval and any requirements pertinent to disposal that are not listed in the license; and (e) a description of the firms current insurance limits for workers' compensation, commercial general liability, and automobile liability.
