- Updated Description for SAM Post � Amendment This notice constitutes Amendment 0001 effective 20AUG2026 to the referenced Request for Quotation (RFQ). The purpose of this amendment is to answer questions. If you have any questions regarding this amendment, please direct them to the contracting office. **Submit Signed Amendment 1 with your quote to be deemed responsible**Attached: Solicitation RFQ Amendment 1 W912PQ26QA0310001 The solicitation W912PQ26QA031 is issued as a Request for Quote (RFQ) from the USP&FO for New York, Purchasing and Contracting Division, Latham, NY. A Firm Fixed Price (FFP) Indefinite Delivery/Indefinite Quantity (IDIQ) contract for Periodic Health Assessments (PHA) throughout New York State as defined in the Performance Work Statement (PWS) for NYARNG MEDCOM. Principle NAICS is 621112 and size standard is $13,500,000.00. This requirement is a total small business set aside. All questions/RFI's must be submitted in writitng to dolan.p.brammer2.civ@army.mil and melissa.santoro2.civ@army.mil no later than 18 August 2026 at 3:00 pm E.T. All quotes must be submitted by email to dolan.p.brammer2.civ@army.mil and melissa.santoro2.civ@army.mil by 27 August 2026 9:00 am E.T. These quotes shall conform to the requirements of 52.212-1 listed in the solicitation. The Government will not accept partial quotes/offers. Award will be made without discussions. Award will be made based on price and other factors shown below. All pricing must be provided, this requirement will not be split. Wage Determination 2015-4187 (Westchester) and Wage Determination 2015-4143 (Albany) applies and is attached. (8) Attachments : 2 Wage Determinations, PWS, Protest Contact Info, Special IDIQ Contract Requirements, Pricing Sheet, Evaluation Factors, and Solicitation. Please include Pricing Sheet with your response. Please sign and return a copy of the RFQ and any/all Amendment(s). In order to be eligible for award, the successful offeror's System for Award Management (SAM) (https://www.sam.gov) registration must be active and includes representations and certifications. The solicitation and all amendments are on www.SAM.gov Contracting Oppurtunities. CAUTION: This solicitation will be issued electronically as will any amendments thereto. Because of this, the Government is under no obligation and is in fact unable to maintain a bidder's mailing list. It is therefore incumbent upon any interested parties to periodically access the above Internet address in order to obtain any amendments that may be issued. Failure to obtain any said amendments and to respond to them prior to the date and time set for receipt of offers may render your offer nonresponsive and result in the rejection of same.
