Description

The United States Department of Agriculture (USDA), Forest Service (FS), Intermountain Region, Bridger-Teton National Forest has a need for decommissioning, removal and replacement of a non-functional natural gas furnace at a Forest Service owned residential facility. Project location is at 421 Black Avenue, Big Piney, WY, 83113. See Attachment 2 � Statement of Work. The following documents are included as attachments to this solicitation and can be accessed via the �Attachments/Links� section of the posting. �Attachment 1 � Solicitation Terms and Conditions. �Attachment 2 � Statement of Work. �Attachment 3 � Schedule of Items. �Attachment 4 � Wage Determination. �Exhibit #1 � Past Performance and Equipment Questionnaire. �Exhibit #2 � SF-1413 Subcontractor Form (If Applicable). AT A MINIMUM, QUOTERS SHALL SUBMIT THE FOLLOWING DOCUMENTS BACK WITH THEIR RESPONSE TO THIS SOLICITATION: 1.Schedule of Items (including Quoter�s Company Information) 2.Certification that repairs can be completed by the date listed in the SOW, and in theDelivery Information Section. See Exhibit #1. 3.Any Amendment that may be issued. 4.Exhibit #1 Past Performance and Equipment Questionnaire. 5.Exhibit #2 SF-1413 Subcontractor Form (If Applicable). 6.Technical Response as Identified in Evaluation and Basis for Award, EvaluationCriteria, Price, Technical Acceptability, and Past Performance.
