Description
The potential lease space selected must be built out for the VA Medical Center to occupy and open a clinic. The Lessor shall procure an Architect/Engineer (AE) to perform a design build for approximately 6500 Net Usable Square Feet for the potential space to be leased. The Lessor and Architect/Engineer shall ensure that the space will meet or exceed the most stringent of the NYS Building Code, NFPA 101 Life Safety Code, NEC and any local applicable building codes. The AE shall incorporate the VA provided preliminary proposed layout in the design phase. This layout shall be used as the basis-of-design starting point. It is expected that through the design phase meetings with the VA, these will be modified to suit the VA needs. Provide labor all new materials, fixtures, tools, equipment and Architect-Engineer (AE) services necessary for design and construction of a fully functional Community based Outpatient Clinic at Carmel, NY. The Lessor shall: 1. Ensure that the Architect/Engineer (AE) performs a design build of the space per the layout approved by VA. a. The AE shall provide the 35%, 65% and 100% submission drawings to the VA COR for approval. b. AE shall provide submittals of all construction materials to be used within the build out of the space to the VA COR for approval. 2. The space shall be built out to include but not limited to the following areas. Refer to the space Configuration plan for the list of proposed rooms. a. Reception Area i. Including sliding windows for patient communication both sides of angled windows ii. Counter space for patients and employees at window iii. One of the windows shall be at an ADA approved height counter. b. Waiting Area to accommodate a minimum of 20 patients and set up for cable TV. b. Vestibule area c. Staff Room with kitchenette i. Including Stainless steel sink and faucet ii. Base and Wall cabinets iii. (3) dedicated 20-amp outlets for small appliances iv. 1 dedicated 20-amp outlet each for refrigerator and Microwave oven. d. ADA restrooms i. Restrooms shall have mirrors installed above sink ii. Exhaust fans shall be installed in restrooms that activate with lights iii. Provide diaper changing station and nurse call system which shall activate an alarm at the reception desk and a strobe light with the words HELP outside the restroom. e. 1 Optometry exam room with 2 lighting controls i. 2 x 2 Fluorescent light fixtures with toggle switch ii. High hat light fixtures with dimming capabilities and dimmer switch f. 1 Visual Fields room i. 2 x 2 Fluorescent light fixtures with toggle switch ii. High hat light fixtures with dimming capabilities and dimmer switch g. Medical and Office supplies Storage Room with shelving h. Phlebotomy Lab Suite with Restroom i. Conference room i. Conference room shall have plumbing roughed in for future sink installations. ii. Build sheetrock wall enclosing the back side of the mailboxes on existing wall in front conference room. j. Medical offices k. Data/Phone Room i. Install dedicated room air conditioner to cool communication equipment maintaining year-round temperature between 68 - 72 degrees F. Room requires cooling year-round due to sensitive equipment. ii. Thermostatically controlled Exhaust fan iii. Shall have a 1-hour fire rated construction rating iv. Shall have a 1-hour fire rated door with self-closer and self-latching hardware l. Hazardous material storage area m. Copy Room and Office supplies storage with shelving n. Janitorial closet i. Closet shall have floor mounted service/mop sink ii. Shall have a wall mounted janitorial faucet installed iii. Shall have a 1 hour fire rated construction rating iv. Shall have a 1-hour fire rated door with self-closer and self-latching hardware v. Thermostatically controlled exhaust with manual by-pass switch o. Medical Exam Rooms p. Women s health Suites q. Dirty room equipped with stand-alone stainless sink with faucets and negative air pressure and shelving r. Clean room with shelving and Positive pressure s. Wheelchair storage and Coat closet with sliding door t. Patient Education and Conference-Video-Telehealth rooms u. Touch down / Swing Office room 3. Provide and install chilled water bottling stations. General Notes THE LANDLORD SHALL BE SOLELY RESPONSIBLE FOR THE MANAGEMENT, PROFESSIONAL DESIGN, AND CONSTRUCTION, INCLUDING ALL LABOR, EQUIPMENT, MATERIALS, AND INSPECTION, TO MEET REQUIREMENTS OF THE CONTRACT. THE AE SHALL FURNISH A DESIGN PACKAGE TO INCLUDE PLANS, DETAILS, SECTIONS, ELEVATIONS, SITE-VISITS AND RELATED INFORMATION AS REQUIRED BY THE CONTRACT. THE DESIGN AND CONSTRUCTION SHALL BE IN ACCORDANCE WITH THE LATEST NFPA 101 LIFE SAFETY CODE, UNIFORM PLUMBING CODE (UPC), NATIONAL ELECTRICAL CODE (NEC), NATIONAL FIRE PROTECTION ASSOCIATION (NFPA) STANDARD, AMERICAN DISABILITY ACT (ADA) COMPLIANCE, VA SPECIFICATIONS AND STANDARDS, AND LOCAL AND CITY CODES. IN THE EVENT THERE IS CONFLICT BETWEEN ANY OF THE STANDARDS/CODES, THE MORE STRINGENT STANDARD/CODE SHALL APPLY. THE AE SHALL HAVE THE CAPABILITY OF COMPUTER-AIDED-DESIGN (CAD). THE CAD PROGRAM USED BY THE AE IN DESIGNING AND DRAFTING THIS PROJECT SHALL BE ABLE TO INTERFACE WITH THE CAD SYSTEM USED BY VA FACILITIES MANAGEMENT/ENGINEERING SERVICE. IN THE FINAL SUBMISSION, THE AE SHALL SUBMIT BOTH HARD COPY DRAWINGS AND CDS CONTAINING ALL DRAWINGS TO THE VA COR. THE AE SHALL USE MICROSOFT WORD TO PREPARE SPECIFICATIONS. IN THE FINAL SUBMISSION, THE AE SHALL SUBMIT HARDCOPY SPECIFICATIONS TO THE VA. THE VA MASTER SPECIFICATIONS MAY BE OBTAINED FROM VA COR (HTTP://WWW.CFM.VA.GOV/TIL/SPEC.ASP). THE VA SHALL PROVIDE THE AE THE CONCEPTUAL DESIGN, (ATTACHED SKETCH), AND BASED UPON THAT, THE AE SHALL SUBMIT A CONSTRUCTION DOCUMENT DESIGN SUBMISSION. THE AE SHALL SUBMIT (1) FULL SIZE, (1) HALF SIZE, AND (1) ELECTRONIC COPY OF THE DRAWINGS AND ONE (1) SET OF EDITED SPECIFICATIONS. THE AE SHALL SUBMIT (1) FULL SIZE AND (1) ELECTRONIC COPY OF THE FINAL AS-BUILT DRAWINGS. THE CONSTUCTION DRAWINGS SHALL BE SIGNED AND SEALED BY PROFESSIONAL ENGINEER.
