Description
PERFORMANCE WORK STATEMENT (PWS) Ship/Activity: SWFPAC Title: Portable Toilet Trailer at NBK-Bangor Building 1006 1. BACKGROUND: Strategic Weapons Facility Pacific (SWFPAC) currently has a bathroom renovation project located at Naval Base Kitsap-Bangor, Building 1006, located in Silverdale, Washington. The renovation project will require the closure of the interior building restrooms, and requires a portable Americans with Disabilities Act (ADA) compliant toilet trailer to be located onsite for a 3-month period. The toilet trailer will also require twice-weekly cleaning service. 2. SCOPE: Toilet trailer shall be ADA compliant and have a minimum number of three (3) toilets with private, gender-neutral or unisex toilet stalls; which lock from the inside and one (1) hand washing station. Note: urinals are not toilets. Each toilet stall must also include a covered container for feminine hygiene product disposal. The toilet trailer will be required to be cleaned a minimum of twice weekly. Twice weekly cleaning must occur between 7:30 AM - 3:00 PM on an every other day or alternating schedule basis, Monday thru Friday, excluding Federal holidays. Cleaning shall include: deep cleaning and disinfection of all interior surfaces, restocking of all consumable supplies (to be provided by the contractor), waste (sewage) removal, and trash removal. Toilet trailer shall be located within a designated distance from Naval Base Kitsap-Bangor Building 1006. See area map (Attachment A) which shows the location of Building 1006 on Naval Base Kitsap -Bangor. See site map (Attachment B) for location details on where the toilet trailer shall be located. 3. PERIOD OF PERFORMANCE: Period of performance to begin within 5 days of contract award for a duration of 3-months, with (3) 1-month option periods of performance which may be exercised at the Government�s discretion. 4. MATERIAL TO BE PROVIDED BY THE GOVERNMENT: Government will provide a 110V power source. 5. MATERIAL TO BE PROVIDED BY THE CONTRACTOR: The contractor shall provide all supplies (including all consumable supplies such as paper products, hand soap, and waste bags), equipment, facilities, personnel, and services required to perform work under this contract. Contractor must also provide electrical power generator and a water storage tank with a capacity to accommodate daily usage for 50 personnel. 6. PLACE OF PERFORMANCE/LOCATION OF WORK: The services under this contract shall be performed on-site located at SWFPAC Building 1006 located at Naval Base Kitsap � Bangor, located in Silverdale, Washington. 7. QUALITY ASSURANCE REQUIREMENTS: The contractor is solely responsible for assessing the quality of the services provided to ensure contract requirements are met. 8. TRAVEL REQUIREMENTS: Contractor must travel to SWFPAC�s facility to perform the scope of work. 9. SPECIAL PROVISIONS/ADDITIONAL INFORMATION: � BASE ACCESS: Contractors must be able to gain access to SWFPAC on Naval Base Kitsap � Bangor, WA. Base access may be granted using the Rapid Gate System, DBIDS System, or other approved base access system at the Contractor�s expense. Once base access is granted, Contractors will be required to apply for a visitor badge (BAVR request) at https://www.bavr.cnic.navy.mil/. Coordination of the visitor badge will be through the Government Point of Contract. � VEHICLE PASS: In order to drive a non-Government vehicle to the work location, the contractor must submit a SWFPAC Vehicle Pass Request and required documents to the Government Point of Contact. The non-Government vehicle must be registered in the company�s name and must have a clear logo displayed on the vehicle indicating the company it is registered to. When entering the base, the vehicle must have the approved vehicle pass displayed in the driver�s side windshield at all times. � SAFETY ISSUES: The work described herein is primarily located in an industrial work area and compliance with safety regulations and protocols is paramount. The Contractor is responsible for maintaining compliance with Occupational Safety and Health Administration (OSHA) requirements for tasks performed, including ensuring personnel are trained for specific tasks and equipped with appropriate personnel protective equipment. � IDENTIFICATION OF CONTRACTOR PERSONNEL: All Contractor personnel are required to identify themselves as such to avoid creating an impression that they are Government personnel. All contractor personnel are required to wear their Government issued badge on the exterior of their clothing, above the waistline, on the front of their body, that clearly identifies who they are. � LANGUAGE IN THE WORKPLACE. All Contractor personnel must be able to read, write, speak, and understand English. All communications and correspondence required by this contract, verbally and in writing, shall be provided in English. � POLITICAL ACTIVITY: Contractor employees shall not engage in any political activity within the Government workplace. � POTENTIAL THREAT: The Contractor must not employ persons for work on this contract if such employee is considered to be a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population. � CONTRACTOR APPEARANCE: Contractor personnel must present a neat appearance and be easily recognized as Contractor employees by displaying their SWFPAC issued badge above the waist line at all times. � OPERATIONS SECURITY (OPSEC): The Contractor is subject to all OPSEC requirements per SWFPAC Instruction 3070.1. � ANTI-TERRORISM/FORCE PROTECTION (AT/FP): The Contractor will be subject to local AT/FP policies and procedures to include emergency evacuation local policy and procedures.
