Description

This is a COMBINED SYNOPSIS/SOLICITATION for commercial items/service prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation number RFQ-25-PHX-028 and the solicitation is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC 2025-03 effective 17 January 2025. This RFP is set-aside for Native American firms IAW Buy Indian Act of 1910 (16 MARCH 2022). The North American Industry Classification System (NAICS) Code for this acquisition is 541519 for Other Computer Related Services with size standard of $34M; Product Service Code (PSC) is R499- Support-Professional: Other. The proposed contract action is to provide non-personal professional MEDIA DESTRUCTION SERVICES FOR THE PHOENIX AREA INDIAN HEALTH SERVICES (PAIHS) LOCATED IN PHOENIX, ARIZONA in accordance with the Statement of Work (SOW). Period of Performance � Estimated base period of 12 MAY 2025 thru 11 MAY 2026. Place of Performance: Phoenix Area Office 40 N. Central Avenue, Suite 605 Phoenix, Arizona 85004 The FAR provisions 52.212-1, Instructions to Offerors-Commercial Products and Commercial Services, applies to this acquisition.
