Description
Could the contracting office provide the incumbent contractor and contract number to better enable our response? There is no incumbent contractor. Page 6 Section B.2, Line Item 5001/5002 lists the 5th ordering period as: May 1, 2029 2028 thru April 30, 2030 Can the government clarify the correct start date for the 5th ordering period (Line Items 5001 5002)? Is it May 1, 2029? Yes, it should read May 1, 2029. Page 8 -""The guaranteed minimum value of services... is $500.00."" ""The maximum value... shall not exceed $500.00."" Can the government confirm the intended maximum contract value, as the solicitation currently lists both the minimum and maximum as $500.00? Yes, this is confirmed. Section 2.1.2.1 Page 11 - Will the VA accept candidates with active audiologist licenses in other U.S. states, or must they be licensed specifically in Louisiana prior to start? Yes. Section E3 2.1 Page 77 - The due date for questions states the deadline for questions as being April 24,2025, the same as the Solicitation Due Date (Page 1 SF25). Will an extension or series of extensions alter the due date while questions are being processed and responded to? There will be no extensions granted at this time. Section 2.1.9 - Can the VA confirm that DEA registration is/ is not required or relevant for any of the proposed Audiologist personnel? A DEA Registration is not typically required among Audiologists as they do not prescribe controlled substances. DEA registration is not required. Section B.3 Page ~18 Is Basic Life Support (BLS) training required to be issued by the American Heart Association (AHA), or are other accredited providers (e.g., Red Cross) acceptable? Yes. BLS, AHA Section B.3 Page ~22 24 Do the required personnel packets need to include documentation of current PPD/TB testing, full immunization records (MMR, Varicella, Tdap, Flu), and background checks for all proposed staff at the time of proposal or prior to onboarding? Immunization will be required within 30 days after the contract award. Section B.3 Page ~19 22 Are staffing agencies required to submit credentialing/personnel packets for Health Technicians (Audiology Assistants)? If so, what must be included? Yes. It must include: a CV/Resume, licenses, certifications, BLS, training or education from an accredited training program. Section B.3 Page ~18 Is ACLS required for any of the proposed positions, given it is listed only in the acronym table? No. BLS only
