Scope The Fort Harrison VA Medical Center (VAMC) intends to award a Fixed Priced contract for: (2) NonRechargeable Battery Stimulators (2) Patient Remote Controls (2) Tined Lead Kit (2) External Trial Stimulators (2) Tined Lead Implant Kits (2) Percutaneous Extensions Brand Name or Equivalent: Line Item Description Unit Quantity 0001 Axonics, NonRechargeable Battery Stimulator/4101 Each 02 0002 Axonics, Patient Remote Control/2301 Each 02 0003 Axonics, Tined Lead Kit/1201 Each 02 0004 Axonics, External Trial Stimulator/1601 Each 02 0005 Axonics, Tined Lead Implant Kit/1801 Each 02 0006 Axonics, Percutaneous Extension/9009 Each 02 Salient Characteristics Shall offer all equipment specified above. Security Requirements The Contractor shall comply with the VA Personal Identity Verification procedures identified in the contract that implement Homeland Security Directive-12 (HSPD-12), Office of Management and Budget (OMB) Guidance M-05-24, and Federal Information Processing Standards Publication (FIPS PUB) Number 201.
