- SF 30 CONTINUATION PAGE 36C25025B0012 AMENDMENT 0004 1. The following answers to requests for information questions are hereby provided to clarify, update, revise, or correct contract specifications and/or drawings. QUESTIONS AND ANSWERS RFI #1 Question: Can the Superintendent be shared across multiple projects at the same site/location, or should they only be dedicated full-time to this project? Answer: Project Superintendent shall be dedicated full-time to this project. RFI #2 Question: Can the Superintendent fulfill the Site Safety & Health Officer (SSHO) role? Answer: Yes. RFI #3 Question: Specification 01 35 26, 1.7 D in its reference to FAR 52.236-6, can be interpreted to mean that the SSHO can also be the Superintendent. Please confirm if the Superintendent can fulfill the Site Safety & Health Officer (SSHO) role? Answer: Yes. RFI #4 Question: If the Superintendent can t be the SSHO, can the SSHO be shared across multiple projects at the same site/location, or should they only be dedicated full-time to this project? Answer: Not applicable, See RFI Question #3 above. RFI #5 Question: Can a subcontractor fulfill the SSHO role? Answer: Project Superintendent must be direct employee of the prime contractor. RFI #6 Question: In the event of contract modifications, should contractors apply bond premiums to the individual modification or wait until the end of the job where the VA issues a final modification to reimburse contractors for bond overruns for all mods. Answer: FAR 52.228-15 (b)(3) shall be applicable to any resultant contract, as Incorporated by Reference in the posted solicitation. RFI #7 Question: Since 2 parking spaces for the prime contractor and all subcontractors is not enough, will the VA allow 2-3 spaces for the prime contractor and each subcontractor for a total of 6 or more parking spaces? Answer: No, only (2) parking spaces in the East Parking structure are permitted per the project specifications. RFI #8 Question: Where shall these people park if the VA cannot make 2-3 spaces available for the prime contractor and each of the subs? Answer: Prime Contractor is responsible for securing off-site parking for subcontractors as required. RFI #9 Question: Is badging required for the project? Answer: Yes, PIV cards are required for all Prime and Subcontractors. Subcontractors who are going to on-site for less than 2 weeks will be issued temporary badges by the COR. Subcontractors who receive the temporary badge need to provide a copy of their OSHA certification to the COR prior to receiving the temporary badge. RFI #10 Question: Are background checks and fingerprinting required? Answer: Yes, background checks and fingerprinting are required. RFI #11 Question: If background checks are required, how long is the turnaround time for background check results? Answer: Varies depending on the VA HR department current workload typical less than 2 weeks. RFI #12 Question: How many hours should the prime contractor and subcontractors budget for on-site badging, safety training, orientation, etc.? Answer: VA cannot provide budgetary hours for onsite badging; Contractor shall cover all costs associated with Badging, Safety Training & Orientation. Safety training and orientation by the VA will occur during the Preconstruction meeting. The Prime Contractor is responsible for managing their own Safety and Orientation requirements. RFI #13 Question: Please confirm if tuberculosis testing is required before contractors can commence work onsite? Answer: Yes, it is required. RFI #14 Question: Will the government provide call-in details and instructions for the bid opening to ensure transparency for all bidders? Answer: Call-in information can be provided upon request. RFI #15 Question: Please provide the point of contact information for the BMS integration contractor? Answer: This information is customarily provided at the post-award/preconstruction kick-off meeting. RFI #16 Question: Please provide the point of contact information for the Fire Alarm integration contractor? Answer: This information is customarily provided at the post-award/preconstruction kick-off meeting. RFI #17 Question: Are there outlets in the work area with power available for contractors to connect to? Answer: Yes, there are standard 120 V duplex receptacle s available for contractor s use during construction. RFI #18 Question: If there are no working outlets in the work area, where will contractors get power from to perform the work? Answer: Not Applicable, see previous question/answer. RFI #19 Question: Will the VA provide space for the Prime Contractor s Superintendent to set up, or should we add costs to our bid for a mobile office? Answer: No, neither a mobile office or interior VA space will be provided for this contract. Site foreman will be able to use area under construction as an office. RFI #20 Question: Will the VA allow a temporary office to be placed in Lot H ? Answer: No. RFI #21 Question: Per the picture below taken from the drawings, please confirm if the prime contractor can use parking lot H for storage, a dumpster, mobile office, as well as parking in the east parking structure. If yes to the question above, please provide a site map to include parking lot H and the east parking structure for the prime contractor parking, temporary office/storage location, and dumpster. If no to the question above, where can the mobile office be located? Answer: Yes, the contractor can use lot H for the dumpster and storage of materials only. See the attached site plan for the lot H location Exhibit A . RFI #22 Question: Is space available for an 8 x 40 mobile office? Answer: No. RFI #23 Question: Are contractors required to set a pole and meter electric usage for the mobile office? Answer: No. RFI #24 Question: Where is power feed available to connect to for the mobile office? Answer: No. RFI #25 Question: Can Portable Toilet be placed near the mobile office? Answer: No. RFI #26 Question: Will the VA provide space for a 30-yard dumpster in Lot H ? Answer: Yes. RFI #27 Question: Can utility tie-ins be performed during regular hours? Answer: Not applicable. See question #17 above. RFI #28 Question: Can system commissioning activities be performed during regular hours? Answer: Yes. RFI #29 Question: Will the VA allow for stored material so that we can procure and lay down long lead items and/or critical path material/equipment? Answer: Yes, the contractor can procure and store long lead time items with a storage container in lot H or within the active construction area. RFI #30 Question: Will the VA fully pay for this material/equipment if stored onsite? Answer: IF stored on-site AND secured the VA will pay for the materials. The VA will not pay for installation until the materials are installed. RFI #31 Question: Please confirm the work hours and schedules called out in the scope of work are correct and that no work is allowed between Midnight to 6am Monday-Friday nor between 5pm to 6am on Saturday & Sunday: Answer: The work hours and schedules called out in the Scope of Work are correct. The shift times are stated explicitly. RFI #32 Question: Will any deviations from these aforementioned work hours and schedules be permitted? Answer: No deviations are permitted. RFI #33 Question: Since room DB37 is outside of the work area, will the shelving and carts in room DB37 outside of the anteroom need to be covered by the contractor or will the VA be covering these carts? Answer: Contractor is required to cover all shelving and carts in room DB37. RFI #34 Question: Since room DB43 is outside of the work area, will the shelving and carts in room DB43 outside of the anteroom need to be covered by the contractor or will the VA be covering these carts? Answer: Contractor is required to cover all shelving and carts in room DB43. RFI #35 Question: Please indicate which, if any, of the existing doors at construction access for Phase 1 (room D25) need to be sealed off. Answer: The door on the west side of the room should be sealed off to create the air balance needs for the ante room per ICRA requirements. RFI #36 Question: Please provide drawings showing the existing fire protection present in the work area. Answer: There are no plans currently showing the location of the existing sprinkler heads. There are (4) existing head within the Phase 1 scope of work area and (6) existing heads within the Phase 2 scope of work area. RFI #37 Question: Will Hazmat suits be required at any point during construction? Answer: Temporary construction notes on page G-100 state that PPE is to be provided by the VAMC. VA will provide: Gowns, foot covers, hair nets & hair nets for facial hair. RFI #38 Question: If Hazmat suits are required, please confirm where, when, and duration. Answer: The PPE is required whenever the Contractor enters or exits the construction areas, refer to note #6 under the ICRA notes on sheet G-100 for additional information. RFI #39 Question: Will the VA provide Hazmat suits? Answer: The VA will provide the required PPE for the durations of the project as noted in item #6 under the ICRA notes on sheet G-100. RFI #40 Question: Will plastic sheeting taped to the walls and floors be sufficient for sealing off the sterilizers and the washers? If not, how should they be sealed off? Answer: Yes, so long as they are secured in place during the duration of the construction phase. RFI #41 Question: Please identify the location of the cart elevator door to be sealed off during construction. Answer: The Cart elevator door opening is on the south side of the cart lift. RFI #42 Question: Is the Door Leaf and Lock called out on DWG A-100 temporary or permanent? Should it be removed after the completion of Phase 1? Answer: It is to be removed after Phase 1 construction. RFI #43 Question: Please provide a schedule/specification for the aforementioned Door Leaf and Lock. Answer: Provide 3 x 7 - 1 3/4"" Hollow metal door leaf, painted with a (3) hinges, door seals, and ADA complaint door level lockset with removeable 7 pin lockset Medeco. Contractor is responsible for field verification of the door size prior to award of contract. RFI #44 Question: Please confirm all existing attached items and equipment to be removed and reinstalled are called out on the drawings. Answer: Confirmed. RFI #45 Question: Please confirm the VA s interpretation of End of Each Work Period as defined in the drawings (Daily, Weekly, Shift Change, etc.). Answer: Contractor is to clean the work area and Ante room of the applicable Phase at the end of each workday. Terminal Cleaning is required at the end of each the Construction Phase. RFI #46 Question: Please specify the type of signage and the locations that will require signage to direct pedestrian traffic away from the construction area. Answer: Provide traffic/ construction signage on the entry door of room D25 at the main Hospital corridor for Phase 1 and provide traffic/ construction signage on the entry door of room D37 at the main Hospital corridor for Phase 2. In addition, provide signage on the inside of the Ante room doors into rooms DB37 & DB43 indicating Do not exit unless emergency . On the outside of the Ante rooms doors from rooms DB37 & DB43 sign should read Do not enter Construction zone.
