- Amendment 0002: Extend the response date. Please sign and return the SF30 for Amendment 0002 with your quote. Amendment 0001: Post Questions and Answers. Please sign and return the SF30 for Amendment 0001 with your quote. The solicitation W912PQ25QA001 is issued as a Request for Quote (RFQ) from the USP&FO for New York, Purchasing and Contracting Division, Latham, New York a Firm Fixed Price (FFP) indefinite delivery/indefinite quantity (IDIQ) contract for Mobile Dental Services throughout New York State (NYS) as defined in the Performance Work Statement (PWS) for NYARNG MEDCOM. Principle NAICS is 621210 and size standard is $9,000,000.00. This acquisition is utilizing procedures in FAR part 12 and FAR part 13.5 Simplified Acquisition Procedures. This requirement is unrestricted. 52.204-26 MUST be included with the quote, if not in sam.gov registration. All questions must be submitted in writing via an email to andrew.e.emrich.civ@army.mil and melissa.santoro2.civ@army.mil no later than 28 February 2025 at 9:00 am ET. All quotes must be submitted by email to andrew.e.emrich.civ@army.mil and melissa.santoro2.civ@army.mil by 10MARCH2025 11:00am ET. These quotes shall conform to the requirements of 52.212-1. The Government will not accept partial quotes/offers. Award will be made without discussions. Award will be made based on price and other factors as shown below. All pricing must be provided for, this requirement will not be split. If something left blank proposal will not be considered. See Addendum to 52.212-2 Evaluation- Commercial Items Wage Determination 2015-5777 Revision: 27 dated 12/23/2024 applies and is attached. Please sign and return a copy of the RFQ and any/all Amendment(s). In order to be eligible for award, the successful offeror's System for Award Management (SAM) (https://www.sam.gov) registration must be active and includes representations and certifications. The solicitation and all amendments are on www.Sam.Gov Contracting Opportunities. CAUTION: This solicitation will be issued electronically as will any amendments thereto. Because of this, the Government is under no obligation and is in fact unable to maintain a bidder's mailing list. It is therefore incumbent upon any interested parties to periodically access the above Internet address in order to obtain any amendments that may be issued. Failure to obtain any said amendments and to respond to them prior to the date and time set for receipt of offers may render your offer nonresponsive and result in the rejection of same.
