CONTINUATION PAGE AMENDMENT 0001 Solicitation Number: 36C78625B0004 Project Description: 863-MM2325-010, Historic Lodge FCA Deficiencies Repairs, Mound City National Cemetery Change 1: Add in its entirety the attached/following solicitation specification VAAR clause: 852.219-75 VA NOTICE OF LIMITATIONS ON SUBCONTRACTING CERTIFICATE OF COMPLIANCE FOR SERVICES AND CONSTRUCTION (Bidder must ensure to complete and submit with their respective bid package, failure to do so will render bid unacceptable, and bid will be rejected.) Change 2: Delete and Replace in its entirety the page 2 of SF 1442 (OFFER) and the Price/Cost Schedule with the attached Price/Cost Schedule rev. Change 3: Add the following text/information to the specification Statement of Work, Scope, General Information section: Historic Architect & Cultural Resources Manager The Mount City National Cemetery is on the National Register of Historic Places. Design will require approval of the NCA Historic Architect: W. Edward Hooker, III (Ed) William.Hooker@va.gov 202-280-8398 (Office Cell) / 251-487-6650 (Personal Cell) Historic Architect & Cultural Resources Manager Department of Veterans Affairs National Cemetery Administration The Historic Architect & Cultural Resources Manager is responsible for coordinating all historic reviews and compliance on behalf of the Department of Veterans Affairs, National Cemetery Administration; and will review and approve all material submitted to be proposed under this contract. The Historic Architect & Cultural Resources Manager does not have the authority to make changes to the contract. The following Request for Information (RFI) questions have been submitted to the Contracting Officer regarding this solicitation. The questions and responses represent all questions submitted to the Contracting Officer prior to the deadline established in the Solicitation. The following questions and responses are provided for bidder s information and use. RFI 1. Window Restoration: For the restoration of the double hung and casement window sash, is the total number of widows to be restored the 15 shown in the photos, or, are there additional windows?� Response: The requirement calls for a total of twelve (12) Lodge Windows to be replaced (see attached photos of four (4) North Windows; four (4) South Windows; and four (4) East Windows; that make up the below Eight (8) Awning Windows and four (4) Fixed Windows. Replace Lodge windows: Eight (8) Awning Windows 60 wide x 32 tall. Replace Lodge windows: Four (4) Fixed Windows 32 x 32 with insulated windows. Note: Original single-glazed, fixed and awning type steel windows are rusted, inoperable, unlockable, and not energy efficient. Rehab and refurbish windows, heads, jambs and sills. (to prevent pest/insect infestation). Replace all twelve (12) storm window screens. Verify and confirm with the COR that the Historic requirements are meet. Note that the requirement has been amended to include an OPTIONAL/ALTERNATIVE BID CLIN for an additional Eight (8) windows: Eight (8) 32 x 70 downstairs (leaded glass) of which interested bidders are required to evaluate and assess for bidding purposes. RFI 2. Bathroom: Is it the VA's intent to install tile at the previous locations on the walls? SECTION 09 30 13 TILING: I see in ""2.1 Tile"" where several different tiles are called out (B - F). Is it the NCA's intent to supply and install similar tile as existing, or is the NCA looking for an updated tile style (i.e. larger format tile for the floor, subway tile for the walls, etc.)? Response: It is the NCA s desire and intent for the Contractor to replace the Bathroom missing tiles with similar/existing matching tiles (color to be preapproved by the COR via the submittal process).
