- PEST CONTROL SOLICITATION # 36C25524Q0070 Questions and Answers Question 1: What is the frequency or schedule of this requirement? This should be according to the Pest Control Schedule (Exhibit 1) in the PWS and part of the final contract. Question 2: How is the frequency or schedule determined? Mainly by priority/importance of areas/departments such as direct patient care areas, for example. Question 3: Is carboard allowed in the facility? As little as possible except where necessary. Cardboard is never allowed in direct patient care areas. Question 4: How often is biohazard removed from the storage area? Is there is a current problem with gnats? If I remember right, this was pertaining to the outside Bio Shed, RMW collection points where it is collected at least once a week. No real problem with gnats. Question 5: How long does it take to complete the entire service? Spraying, prevention, etc. The vendor will determine how and when to spray areas according to the Pest Control Schedule (Exhibit 1), the frequency listed and their visitation schedule laid out in the PWS. Question 6: What is the frequency of the service? In the PWS. Question 7: What are the main problem areas/concerns? The main facility is a very old building with a many small openings throughout. Engineering tries to address that, but as I said it is a very old building. The sightings reported the most are spiders. Question 8: How soon are invoices paid? I certify all invoices on our contracts soon after receiving them through our invoicing system. When they are paid would be a finance question. Question 9: How often are there emergency services? The expected frequency is listed in the PWS/contract. Question 10: What is the total square footage of the service request? We are a sizeable campus. I believe with the main facility plus all the outlying buildings, we are looking at in the ballpark of 500,000 square feet. This will grow considerably over the next 0-3 years with extra sections and buildings being added to the campus. Question 11: How many buildings/locations are there to service? Please see Pest Control Schedule (Exhibit 1). Question 12: What is the schedule for the service? Spraying etc. Required visits are according to the PWS and frequency is according to the Pest Control Schedule (Exhibit 1). Question 13: Who do we email? At this point, it should be the Contracting Officer. Question 14: What was the previous awarded Solicitation Number, Date awarded, and awarded value? We don t supply that information. Question 15: Has any part of the Statement of Work (SOW) changed in this solicitation compared to the last awarded contract? If so, what changed or was added? Please read the statement of work and give us your quote for the stated need. Question 15: Has the government issued any CURE or Notice of Correction for pest control services over the past 24 months at this location? That information isn t needed to provide a quote. Question 16: Will the government require SDVOSB Prime Contractors themself to have relevant pest control past experience including pesticide applications for the best interest of public health & safety and in alliance of the VHA Directive 1850.02 Pest Management Operations and VAAM Appendix M815-A Department of Veterans Affairs Source Selection Guide? Yes, the SDVOSB must have past contracts that they have done at least 51% of the work according to Limitations on Subcontracting. Question 16: Can the government clarify how many emergency service requests there have been in the past 24 months? Number wasn t kept on record. Question 17: Can the government confirm the number of exterior rodent bait stations and interior rodent traps that are present? Will the government require these to be provided by the contractor upon� award.� Per SOW: Exterior rodent bait stations = Approx. 12 & Interior Rodent traps = 14 Question 18: Can the government clarify how many fly lights are being used at the Marion VAMC? Approximately 20. Question 19: Can the government provide the complete addresses for the following facilities as they require service on an as needed basis for Bed Bugs: a.� � � � � Evansville Indiana Patient Clinic Evansville Health Care Center 6211 E. Waterford Blvd Evansville, IN 47715 b.� � � � � Owensboro Kentucky Patient Clinic Owensboro Community Based Outpatient Clinic 2060 East Parrish Avenue Owensboro, KY 42303 Question 20: Can the government clarify in the Price/Cost Schedule Item Number 0001, 1001, 2001, 3001, 4001 Indicate 0.00 as Quantity and HR as Unit. Will this Price/Cost Schedule be updated Quantity to reflect against Monthly or Yearly Pricing? This is based on vendors quotes.
