SOURCES SOUGHT The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 8), James A. Haley Veterans Hospital, Tampa, FL is currently seeking information from firms, Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, Hub zone, 8(A), Woman Owned Small Business OR Small Business with the ability to provide these following services and requirements listed below: Statement of Need MEDICUS VIABLE BONE ALLOGRAFT GENERAL:  The Neurosurgery Operating Room of the James A. Haley Veterans Hospital, 13000 Bruce B Downs Blvd, Tampa, FL 33612, has a requirement for the Medicus Viable Bone Allograft Plus 5cc and 10cc.  Vivex Biologics, Inc. says the Medicus Viable Bone Allograft Plus is an advanced bone graft that preserves native lineage-committed bone forming cells that promote bone fusion.   The Medicus Viable Bone Allograft Plus is used frequently in Neurosurgery Spine cases. BACKGROUND: The Neurosurgery Spine physicians use this viable bone graft for most spine cases that involve multilevel fusions of the spine. Medicus Viable Bone Allograft Plus is designed for the repair, replacement, or reconstruction of bone defects. The 5cc and 10cc versions of Medicus Viable Bone Allograft Plus will be specifically used in the Operating Room for neurosurgery spine cases. This product is aimed at providing optimal outcomes for veterans. PHYSICAL CHARACTERISTICS: Supplies must have the physical characteristics to enable the government to perform the capabilities. This product is a biologic frozen implant that is stored in the Operating Room Freezer at -82 degrees and thawed at time of use. Medicus Viable Bone Allograft Plus consists of three components: a bone particulate component, a bone gel component, and a cell component. The bone particulate and bone gel components are derived from mineralized and demineralized bone particulates. The bone particulate component is lyophilized and provided in a sterile manner. The cell component is preserved frozen using a 100% polyampholyte-based cryoprotectant. SPECIFIC NEED/SALIENT CHARACTERISTIC S: Additionally, the Medicus IFU 80-662 Rev. 01 says Medicus Viable Bone Allograft Plus is intended for use for the repair, replacement, or reconstruction of osseous defects. The Medicus Viable Bone Allograft Plus 5cc and 10cc will be specifically used in the Operating Room for Neurosurgery Spine cases. This product provides the best outcome for veterans that require spine surgery with fusion. Manufacturer: Vivex Biologics, Inc. Haley Veterans Hospital, 13000 Bruce B. Downs Blvd, Tampa, FL 33612 Specific Delivery Information: Building 1, Warehouse (Attention Javier Tapia and Mark Hunter Logistics) DELIVERY SCHEDULE: All items to be delivered within 30 days of purchase order processing and order placement.  SPECIAL REQUIREMENTS: All work shall be performed by fully qualified personnel. Employees must be aware that work is in a hospital environment and courtesy must be extended to patients and visitors. Work shall be coordinated to minimize impact to hospital operations. The applicable North American Industry Classification System Code (NAICS) for this procurement is 339113.  To be considered a small business under this code the SBA size standard of 800 employees.
