Description
NOTICE OF INTENT TO SOLE SOURCE: IAW FAR 5.101 this notice is to advise the public that The Department of Veterans Affairs, Regional Procurement Office East (RPOE), Network Contracting Office 4, intends to award a sole source, Firm-Fixed-Price contract pursuant to FAR 13.106-1(b) to Chemdaq Inc. located at 300 Business Center Drive, Pittsburgh, PA, 15205 for a base year with four (4) option years, exercisable at the Government s discretion. The Altoona VA Medical Center (VAMC) requires Steri-Trac Hydrogen Peroxide and Peracetic Acid sensor exchanges for three Chemdaq monitoring systems at the facility. The monitoring systems ensure the safety of sterile processing service staff during low temperature sterilization processes. THIS NOTICE OF INTENT IS NOT A REQUEST FOR COMPETITIVE PROPOSALS or QUOTATIONS. Responsible sources that believe they can meet the requirements may give written notification by submitting capability information that clearly demonstrates the capability to perform the required work and applicable certifications/licenses for the Hardware/Software. Capability information is required prior to the response due date and time, to establish the ability of the source to meet this requirement. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to perform the requirements and must include relevant corporate experience along with the applicable contract number, total contract dollar amount, contract period of performance, details of the relevant services and customer point of contact with corresponding telephone number and email address. Responses must include following information: 1. Company Name 2. SAM number 3. Company s address 4. Point of Contact information (title, phone number and email address) 5. Business size. A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. The Government will not be responsible for any costs associated with the preparation of responses to this notification. Responses are due by 4:30pm ET, Friday, November 08, 2024. No submissions will be accepted after this date and time. Responses shall be submitted via email to jefferson.mann@va.gov. Telephone inquiries will not be accepted. The NAICS code for this acquisition is 811210 and the size standard is $34 Million. Statement of Work (SOW) ChemDaq Monitoring System Steri-Trac Sensor Usage/Exchange Contractor shall provide Steri-Trac Hydrogen Peroxide (H2O2) and Peracetic Acid (PAA) sensor service/exchange for ChemDaq monitoring equipment for the James E. Van Zandt Medical Center (JEVZMC). All services shall be provided in accordance with the following Statement of Work. Location: James E. Van Zandt, VA Medical Center 2907 Pleasant Valley Blvd. Altoona, PA 16602 Purpose: Contract is needed to continue the Steri-Trac sensor exchange service of the Peracetic Acid and Hydrogen Peroxide monitoring systems from ChemDaq already in use at JEVZMC. The system ensures the safety of staff by monitoring hydrogen peroxide and peracetic acid levels used during the low temperature sterilization process in the Sterile Processing Service area. Sensors used beyond the calibration expiration date could cause dangerous conditions for staff. SERVICES: Contractor shall furnish calibrated sensors for the following Steri-Trac monitoring systems: Steri-Trac2 (H202) Monitor (serial # AM-230928-13), Steri-Trac (H202) Monitor (serial # AM-230928-14) and Steri-Trac (PAA) Monitor (serial # AM-230928-15). Required Tasks -Provide two H202 and one PAA replacement E-Cell sensors when sensors need recalibration (9 total per year), for the term of the contract. -ChemDaq will track to ensure new sensors are sent every four months (three times a calendar year) to the attention of the Biomedical section at JEVZMC. -Notify Biomedical section of the shipping details and provide JEVZMC with a pre-paid return shipping label to return used sensors. -Provide telephone support for monitor/sensor troubleshooting during normal business hours (Monday through Friday from 8am-5PM), excluding federal holidays. SECURITY REQUIREMENTS: The POC is responsible for ensuring that all Contractors comply with physical security policies. A compliant ID badge must be worn by Contractors always while on VA premises. The Vendor is required to comply with physical security guidelines by either checking in with the VA Police when they come on-site to perform contracted services, or by obtaining a VA Contractor ID badge from the VA Police. Contractor may not have access to the VA network or any VA sensitive information under this contract. Contractor must always be escorted by an authorized employee while performing work in sensitive OIT areas. All Contractors must receive Privacy training annually using one of the following methods: Complete VA Privacy Training for Personnel without Access to VA Computer Systems or Direct Access to or Use to VA Sensitive Information training by using VA s TMS system (https://www.tms.va.gov/). Contractors may use the TMS Managed Self Enrollment method to complete the training in TMS. The POC must ensure that all contractors are validated in the PHI domain. Complete the hard copy version of VA Privacy Training for Personnel without Access to VA Computers. PERIOD OF PERFORMANCE: The period of performance will be one year from date of award with four one-year option periods, executable at the Government s discretion.
