Special Notice Notice of Intent to Sole Source This is a Special Notice as a Pre-Intent to Sole Source. The Department of Veteran Affairs, Veterans Health Administration, Network Contracting Office 16, Houston, Texas 77056, intends to negotiate a sole source Firm-Fixed Price contract for a performance period of a base period of one (1) year, with four (4) additional, 1-year ordering periods to Olympus America Inc., located at 3500 Corporate PKWY, Center Valley, Pennsylvania,18034, for Scope testing and maintenance at the Michael E. DeBakey VA Medical Center (MEDVAMC) located at 2002 Holcombe Boulevard, Houston, TX 77030. It has been determined that through initial Market Research, only one (1) responsible source can provide the required Brand Name supplies in accordance with Federal Acquisition Regulation (FAR) 6.302-1 Only one responsible source and no other supplies or services will satisfy agency requirements . Please review the attached document(s) for more details on this requirement. 36C25625Q0040 SOW NO SOLICITATION EXISTS AT THIS TIME. Please provide the information you deem relevant in order to respond to the specific inquiries of the notice. Information provided will be used solely by MEDVAMC as Market Research and will not be released outside of the MEDVAMC Purchasing and Contract Team. RFI responses are due by 1500 pm Central Standard Time on October 22, 2024; size is limited to 8.5 x 11 inches, 12-point font, with 1-inch margins in Microsoft Word format via email to Frank Sanders at frank.sanders1@va.gov. The subject line shall read: 36C25625Q0040 Response to Intent to Sole Source. Special Notice Notice of Intent (NOI) to Sole Source The Department of Veterans Affairs Michael E. DeBakey VA Medical Center (MEDVAMC) Network Contracting Office (NCO) 16 intends to issue and award a Sole Source single award Firm-Fixed Price type contract with a performance period of a base period of one (1) year, with four (4) additional, 1-year ordering periods to Olympus America Inc., located at 3500 Corporate PKWY, Center Valley, Pennsylvania,18034, for Scope testing at the MEDVAMC, located at 2002 Holcombe Boulevard, Houston, TX 77030. Market research has determined that these items are highly specialized, are only available from the original source without an unacceptable delay or duplication of cost and no other type of items will satisfy agency requirements. The Government intends to award the contract with an anticipated date of January 1, 2025, to Olympus America Inc., 3500 Corporate Parkway, Center Valley, PA 18034 CAGE: 32212. The intended procurement will be classified under North America Industry Classification System (NAICS) 339112 - Surgical and Medical Instrument Manufacturing with a Large business size standard of 1000 employees. Telephone requests will not be honored. No award will be made based on quotes or offers received in response to this notice. However, interested parties may identify their interest and capability to respond to this requirement no later than the date/time indicated below. A determination not to compete this requirement, based upon responses to this notice, is solely within the discretion of the Government. Information received will normally be considered solely for determining whether to conduct a competitive procurement. The Department of Veterans Affairs will NOT be responsible for any costs incurred by interested parties in responding to this notice of intent. Only written responses will be considered. All responsible sources may identify their interest in meeting this requirement by sending their email to Frank Sanders by 1500 pm Central Standard Time (CST), on October 22, 2024. Additionally, any response to this notice must show clear, compelling, and convincing documentation demonstrating their capabilities to satisfy the requirements that competition will be advantageous to the government by providing the following: 1. Business Size and Socio-Economic Status: (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a Small Business Administration (SBA) Veterans Small Business Certification (VetCert) certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the Unique Entity Identification (UEI) number of your firm. (h) State whether your firm is registered with the System for Award Management (SAM) at https://sam.gov and the VetCert Registry at https://veterans.certify.sba.gov (if applicable). If not, please NOTE: any future solicitation could only be awarded to a contractor who is registered in SAM. To receive award based on SDVOSB or VOSB status you must be certified in the VetCert Registry. All responses from responsible sources will be considered. As a result of analyzing responses to this notice of intent, the Government shall determine if a solicitation will be issued. Any prospective contractor must be registered in the SAM to be eligible for award. 2. Authorization to Perform: Prospective, non-Original Equipment Manufacturer (OEM), responses to this notice MUST provide a distributor letter from the OEM-on-OEM letterhead as an approved OEM authorized distributor identifying vendor is an approved distributor to provide the required OEM parts, supplies, equipment as described herein this notice. The capability documentation should include general information and technical background describing in detail the prospective contractor's capabilities and experience that will demonstrate the ability to support these requirements, and verification by an authorization letter from the OEM-on-OEM letterhead. The intent of this notice is for informational purposes only.
