Page 9 of 9 Number: 36C10G24Q0077 Notice Type: Combined Synopsis/Solicitation Synopsis: COMBINED SYNOPSIS/SOLICITATION This is a combined synopsis/solicitation for commercial items prepared by the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number 36C10G24Q0077 is issued for this combined synopsis/solicitation; this solicitation is being issued as a Request for Quote (RFQ). This combined synopsis/solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2024-05, dated 22 May 2024. Requirement Description: The Veterans Health Administration (VHA) Equipment Life Cycle Management (ELCM) Program identified Optometry Exam Chair and Instrument Stands as candidates for a VA-wide (otherwise referred to as national ) single award Requirements contract. Optometry Exam Chairs and Instrument Stands are a full power articulating examination chair suite suitable for routine practice and minor procedures. The chair can recline to a nearly flat position and has an adjustable surgical headrest. Most chairs are dual-sided operations, which allows practitioners to control the chair from either side while maintaining patient contact. The upholstered armrests and a large, hinged footrest add to patient stability and comfort. The chair also comes with a surgical headrest that facilitates eye examination and other eye procedures. The ophthalmic equipment stands are specifically designed to hold several ophthalmic testing devices, to enhance the overall eye care experience of patients while ensuring a consistent and simple ease of use for medical practitioners. Optometry chair and stand units provide an integrated eye care lane that includes both the chair and the stand. A Brand Name or Equal (BNOE) single award Requirements contract will be awarded IAW FAR 13.303 by all terms, conditions, provisions, specifications, and schedule of this solicitation herein. The associated North American Industrial Classification System (NAICS) code for this procurement is 339115 Ophthalmic Goods Manufacturing: 6540 Ophthalmic Instruments, Equipment, and Supplies and the associated size standard is 1,000 employees. This procurement action is issued as a Full and Open Competition. The Combined Synopsis/Solicitation Notice #36C10G24Q0077 NX EQ Optometry Exam Chairs and Instrument Stands is hereby amended to change the following: All interested companies shall provide a quote for all Contract Line-Item Numbers (CLINs) listed in the amended ATTACHMENT A PRICE COST SCHEDULE - NX EQ MANUAL OPTOMETRY EXAM CHAIRS AND INSTRUMENT STANDS. SubCLINs 0001AA 0001-0001AM is for color only and pricing is for CLIN 0001. Quoters can disregard sending ATTACHMENT F PAST PERFORMANCE REFERENCE AND ATTACHMENT G PAST PERFORMANCE QUESTIONNAIRE documents in their quote proposal submissions. Not required. Incorporating an update to D.7 52.212-2 EVALUATION COMMERCIAL ITEMS (NOV 2021), C. EVALUATION APPROACH. Adding C.3 as an evaluation approach for Veterans Preference. In accordance with ATTACHMENT B Contract Administration/Invoice Instructions, B.3 Acknowledgement and ATTACHMENT D Solicitation Provisions, Volume IV, acknowledge amendment at time of quote proposal submission. Quotes are due via email by 11:59 PM EST on August 7, 2024, to Contract Specialist, Lisa Thompson at Lisa.thompson10@va.gov and Contracting Officer, Hosea Berkley at Hosea.Berkley@va.gov remains the same.
