The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI 53214, is performing market research to determine if there is a sufficient number of qualified: (1) Service Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); or (3) Small or emerging small business firms, or (4) Other Than Small business firms, to provide brand name or equal SRS Medical EasyFlo Uroflow System at the Clement J. Zablocki VA Medical Center. General Requirements: BRAND NAME OR EQUAL Six (6) , SRS Medical EasyFlo Uroflow System (Item number is EF) and its accessories: a) The dimensions of the printer (assembled) must not exceed 16 inches L x 10 inches W x 8 inches D b) Must have universal input power supply;EN60601-1 marked c) Must be stand-alone uroflowmetry system d) Must have a simple touch pad operation e) Must be able to record: - Urine flow pattern (graphically) - Total voided volume - Total voided time - Urine Flow Time - Average urine flow - Maximum (peak) flow - Onset-to-peak flow time - Patient comparison Study f) The device must start automatically with urine flow and shut off within 30 seconds once urine flow is no longer detected. g) Must be able to print out patient's results into the liverpool nomogram for comparison. g) Must include six (6), 6 rolls/box of thermal paper (Item number is 84411305) h) Must include One-Year Warranty (Part and Labor) VAAR 852.212-71 GRAY MARKET ITEMS will be applicable to any request for quote for this item.
