- Description of Amendment: The purpose of this amendment is to: Provide answers to Vendors questions. update the salient characteristics deleting characteristics that apply only to mobile surgical lights component and included additional clarifying language added to the installation component. All changes are bolded in Red. Update the Buy America Act Clause and Provision to reflect the correct Clause 52.225-3 (Buy American-Free Trade Agreements-Israeli Trade Act NOV 2023) and attached Provision 52.225-4 Buy American-Free Trade Agreements-Israeli Trade Act Certificate NOV 2023 (to be completed by Vendor). The quote due date is extended by Two (2) days and quotes are now due 09/11/2024 @ 15:00 PT. All other terms and conditions remain unchanged. Questions: What level of Infection Control Risk Assessment (ICRA) is required? Because it is a non-sterile area outside the ORs, we believe this should be a level 1 or 2 ICRA with terminal cleaning. Please confirm. Level 1: Inspection and Non-Invasive Activities or Level 2: Small scale, short duration activities which create minimal dust. This will depend on if dust or debris is created by their work. I think since they're replacing the light, there will be some drilling and minor debris produced, which is category B work. The OR area is a ""highest risk"" area. So based on this risk matrix, it will be Class II or III. Is the local electrician connecting and disconnecting the surgical lights or does the vendor need to provide an electrical contractor? Contractor shall provide electrician for disconnecting and connecting surgical lights. Can both ORs be shut down at the same time to expedite the installation? No only one can be shut down at a time What is the brand of equipment/lights to be removed? AMSO ceiling mounted lights from 1988. Salient Characteristics: Brand name or equal to Draeger Polaris 600 Surgical Lights with wall control (PN: G36402). The following characteristics are essential for the physical, functional, and/or performance of this requirement. 92 LEDs 3800K, 4400K, 5000K and 5600K color temperature settings 360-degree light head rotation for clinical maneuverability Ambient light and light guide feature built in SWKI Class 5 laminar flow compliance IP53 ingress protection from dust and water Medical grade glass surgical lens material LED lifetime average 50,000 hours 20,000-160,000 lux light intensity 3000 lx ambient light intensity (3.000 Lux) Lightweight Mounting arm lengths can be somewhat customized to the size and configuration of each operating room. Can be mounted to existing spindle from ceiling. 24 hour battery charging time Item is for mobile surgical lights can be excluded 180 minute bridging time Item is for mobile surgical lights can be excluded Sterile handles for cleaning Compatibility Can be mounted to existing spindle from ceiling using existing electrical hookups. Dimensions operating room dimensions vary for mounting considerations. Installation: The Contractor shall provide all necessary travel, labor, materials, tools, documentation, and parts required for the removal/disposal of the current light fixture and the install of the new light fixture. At a minimum, the Contractor shall provide two (2) certified technicians for a minimum of One (1) day to safely remove/dispose of current lighting fixture/system and install the new light fixture IAW manufacturer s guidelines. Contractor shall ensure the new lighting system is completely setup for the physical, functional, and/or performance of the procured lighting system IAW the manufacturers guidelines. The Contractor shall: Permanently remove the old item and install, in its place, a new and unused item which is identical in material, form, fit, and function. The new item shall: Have the same ability as the old item, to physically interface or interconnect to existing spindle and electrical wiring. Contractor shall provide electrician to perform removal of old lighting fixtures and installation and electrical hookup of new lighting fixtures. The new lighting fixtures shall perform the same action or actions of the original item was designed to perform. Contractor shall ensure new lighting fixtures work (turn on/off and maneuver in accordance with technical aspects of purchased lighting fixtures). Biomedical Point of Contact (provided at award) shall witness and verify that form, fit, & functionality is performed and desired performance is achieved at time of install. Delivery shall be FOB: Destination: 30-60 days after receipt of order is preferred. All deliveries will be coordinated with the Puget Sound Health Care System Point of Contact.
