***************** AMENDMENT 0001 - QUESTIONS and ANSWERS ****************** 1. The purpose for this amendment is to answer questions received and attach a revised Performance Work Statement to incorporate the Quality Assurance Surveillance Plan. 2. QUESTIONS and ANSWERS: a).�Question 1: �For the equipment that needs repairs, how will we charge for parts? I noticed there is not a section for costs of parts for repairs, so I just wanted to have this clarified.� ANSWER: A line item for the repair parts will be added to the Call/Order, as needed. b).�Question 2: �Is there any incumbent/recompete or this is a brand-new project? If yes, could you please provide us with their�award number or even if the customer�bought similar services in the past.� ANSWER: No incumbent for this requirement. c).�Question 3: �Could you provide more detailed information on the specific types of fitness equipment that require maintenance and repair?� ANSWER: Cable Machines, Smith Machines, Treadmill, Elliptical, Row Machine, and Stationary Bicycles. d).�Question 4: �Are there any specific brands or models of equipment that we should be familiar with? ANSWER: Life Fitness and Concept II. e).�Question 5: �How many OKARNG locations will require service, and where are these locations geographically?� ANSWER: There are approximately 25 locations. A list of the location is attached with the documents in this solicitation. f).�Question 6: �Can you provide more detail on what is expected in the Technical Proposal?� ANSWER: See paragraph 5 of the attachment �52.212-1 Instructions to Offerors for Proposal Submittal, Addendum�. g).�Question 7: �Are there any specific qualifications or certifications required for personnel performing the work?� ANSWER: Qualifications required in accordance with the commercial industry standard. The Government is not requiring additional qualifications to perform the maintenance or repair services for the equipment. h).�Question 8: �Will past performance be evaluated, and if so, what specific past projects or experience should be highlighted?� ANSWER: Past performance will not be evaluated. Should the Offeror�s company have an �Unfavorable Determination of Responsibility�, per paragraph 4 of the attachment �52.212-1 Instructions to Offerors for Proposal Submittal, Addendum�, the proposal will not be technically evaluated for award. i).�Question 9: �Will there be an opportunity to conduct a site visit before submitting a proposal?� ANSWER: Site visits will not be conducted. j).�Question 10: �Are there any performance metrics or standards that will be used to evaluate the success of the services provided?� ANSWER: The Quality Assurance Surveillance Plan will be attached put in the revised Performance Work Statement. k).�Question 11: �What is the expected duration of the Blanket Purchase Agreement (BPA)?� ANSWER: See paragraph 1.3 of the Performance Work Statement. l).�Question 12: �Can you clarify how proposals will be evaluated under the Lowest Price Technically Acceptable (LPTA) criteria?� ANSWER: Will be evaluated as �Acceptable� or �Unacceptable�, per the attached �52.212-1 Instructions to Offerors for Proposal Submittal, Addendum�. m).�Question 13: What are the specific technical acceptability standards? ANSWER: See paragraph 6 of the attached �52.212-1 Instructions to Offerors for Proposal Submittal, Addendum�. 3. The proposal due date has been extended to 16 Sep 2024 by 1:00 p.m. CDT. ************** END OF AMENDMENT 0001 ***************** 1. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 2. The solicitation number is W912L6-24-R-5070 issued as a Request For Proposal (RFP) in accordance with the requirements of Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, using the procedures of FAR Part 13, Simplified Acquisition Procedures. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2024-05 dated May 22, 2024. The FAC and FAR are available at https://www.acquisition.gov. 3. This solicitation is a Small Business Set-Aside with the North American Industry Classification System (NAICS) code 811310, Commercial and Industrial Machinery and Equipment (except Automotive and Electronic) Repair and Maintenance with a standard size of $12.5M and Product and Service Code (PSC) J078, Maint/Repair/Rebuild of Equipment�Recreational and Athletic Equipment. 4. The Government will not reimburse any costs incurred or associated with in relation to the preparation of a proposal for this solicitation, W912L6-24-R-5070 or if the RFP is canceled. 5. It is the Government�s intent to award an �All or None� Firm Fixed Price (FFP) Blanket Purchase Agreement (BPA) based on the Lowest Price Technically Acceptable (LPTA) for the maintenance and repair of the Fitness Equipment for the Oklahoma Army National Guard (OKARNG) without discussion. 6. The contractor shall provide all tools, supplies, equipment, transportation, labor, supervision, and management necessary to support, maintain, and repair the fitness equipment at all the OKARNG locations. Contractor is required to respond to the Technical Proposal and Price Proposal criteria located in the attached 52.212-1, Instructions to Offeror � Addendum. 7. Delivery and Acceptance will be FOB Destination, per service location stated in the PWS. 8. Provision 52.212-1, Instructions to Offerors-Commercial Products and Commercial Services applies to this acquisition, to include any addenda to the provision. Provisions and Clauses are attached to include the technical evaluation criteria for award. 9. The Offeror is required to have a current and active registration in the System for Award Management (SAM) website, https://sam.gov at the time the proposal is submitted and not in a �pending� status, as well as no exclusions. 10.
