Description
SOURCES SOUGHT SYNOPSIS The Department of Veterans Affairs is issuing this sources sought synopsis as a means of conducting market research to identify firms having an interest in and the resources to support a requirement to establish a service maintenance contract for two Candela Lasers located at the Clement J. Zablocki VA Medical Center, located at The result of this market research may contribute to determining the method of procurement. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 811210 Electronic and Precision Equipment Repair and Maintenance (i.e., not combined with any other service). THERE IS NO SOLICITATION AT THIS TIME. This request for capability information does not constitute a request for proposals; submission of any information in response to this market survey is purely voluntary; the government assumes no financial responsibility for any costs incurred. Tentative requirements: PERFORMANCE WORK STATEMENT CANDELA LASER SERVICE CONTRACT Description of Services The Contractor shall provide all labor, materials, tools, travel, management, and equipment necessary to provide service maintenance of the Candela Lasers at the Clement J. Zablocki VA Medical Center (Zablocki VAMC), located at 5000 W National Ave, Milwaukee, WI 53295-0001. Scope of Work This service contract is for an initial one-year term but may be renewed annually for up to 4 years. It provides preventative maintenance (PM) on the specified equipment, as well as labor, travel, expenses, and parts for that preventative maintenance. Parts for PM shall include, but are not limited to: PM kits, dyes, and consumables required to perform PM. Additionally, it provides on-call corrective maintenance on equipment that is malfunctioning or inoperable, as well as all labor, travel, expenses, and parts deemed necessary for that corrective maintenance. 2.A) Equipment: Scheduled of Work Scheduled work shall be required in the performance of this contract and shall be performed during the hospital s normal business hours of 8:30 a.m. and 5:00 p.m., Dispatch and Tech Support: Monday through Friday, 7:30AM to 7:00PM CST (Wisconsin), and Contractors shall respond on-site as required within 48 hours of service request/notification, excluding weekends and National Holidays, to include any other day specifically declared by the President of the United States to be a Federal Holiday. National Holidays observed by the Zablocki VAMC are: New Years Day Independence Day Labor Day Thanksgiving Day Memorial Day Christmas Day When one of the above designated Federal Holidays falls on a Sunday, the following Monday shall be observed as a Federal Holiday. When a Federal Holiday falls on a Saturday, the preceding Friday shall be observed as a Federal Holiday by United States Government agencies. If different times shall be needed, this shall be discussed and mutually agreed upon with the Zablocki VAMC Contracting Officers Representative (COR) and shall still be considered normal business hours. No work by the Contractor on Federal Holidays and no work on the weekend when the Federal Holiday is on the respective Monday or Friday shall be done. The Contractor shall obtain prior approval at least 24 hours from the Zablocki VAMC s COR. Specifications of Work: All parts for the repair and corrective maintenance of the system are covered No exclusion or charge for repairs due to damage/abuse. Engineering changes are covered. Annual Preventative Maintenance (PM) according to manufacturer specifications. PM s shall be completed in the months as required by local Biomedical Engineering staff. Unlimited corrective maintenance is included during hours of coverage. Any required parts shall be shipped in an expedited manner to minimize equipment downtime. Software upgrades shall be included. No charge if equipment is not immediately available to field service engineer upon arrival at facility. Guaranteed Uptime: This plan includes a guarantee of 95% uptime. Reporting Requirements The check-in is mandatory at 5000 W National Ave. Milwaukee, WI 53295-0001, Building 70: C Wing prior to performance of service. The contractor's Field Service employee shall wear visible identification at all times while on the premises of the Zablocki VAMC. The contractor's Field Service reports shall be provided after the completion of each field service call. Field Service reports shall include written evidence of performance of all procedures and tests as required by the manufacturer. They shall include itemized parts and labor. The Contractor shall provide certification that all test equipment is calibrated annual by an ISO 9001 certified provider. Certification documentation shall be provided upon request within 10 business days. The government will evaluate market information to ascertain potential market capacity to provide services consistent in scope and scale with those described in this notice and otherwise anticipated.
