- Evaluation Factors Factor 1 - Technical Provide a detailed plan for meeting the salient characteristics as stated in SOW. Factor 2 - Past Performance Offerors must provide evidence of their relevant experience performing the work described. Identify Federal, State, and Local Government and private contracts that are similar in scope and size. References should include no more than three (3) references of on-going contracts or those completed within the last three (3) years. Include information that depicts contract compliance, financial stability and efficiency, and customer satisfaction. The list of references should include the following at a minimum: Name of Reference/Business and the Contract Number Contact person name and phone number and email address Factor 3 Price The Price Proposal will be evaluated using the Individual Price Schedule submitted by offerors. 1. Quote Preparation Instructions: a. Specific Instructions: Submit signed and dated quote to Contract Specialist Tyrone Whalen via email to tyrone.whalen@va.gov. All quotes shall be legible and prepared in the following general format below to be properly evaluated. Quotes shall be complete, self-sufficient, and respond directly to the requirements of this solicitation. Email size limit is 10 MB. If needed, large attachments may be sent over several emails. b. Submission Format: ** ALL VENDORS ARE REQUIRED TO SUBMIT PARTS 1 THROUGH 4 BELOW TO BE CONSIDERED RESPONSIVE ON THIS RFQ SOLICITATION.** Part I: Executed Quote (Signed SF1449) Part II: Price Part III: Technical (see page limit and formatting specifics below) Part IV: Past Performance i. Part I Executed Quote: Complete SF1449 which includes filling out 17a, 30a-c, B.1 information (Amendment Acknowledgement) and E.10 52.212-3 (as applicable and instructed per directions in the provision). ii. Part II - Price: Insert proposed unit and extended prices in Contract Line Item Numbers (CLIN) 0001 through 4019. All prices shall be represented in two decimal positions only. Example: $0.27, not $0.27458. Ensure total calculations are correct when quantity is multiplied by unit price. iii. Part III Technical. Page limit for Technical submission is 15 pages in Times New Roman font size 12 with 1 inch margins. Page limit includes does not include any cover letter and/or table of contents for this specific submission. Offerors shall submit the following: 1. Technical approach: Provide a plan that addresses each part of Factor 1 - Technical. iv. Part IV Past Performance 1. Identify all federal, state, and local government contracts as well as any private/commercial contracts of similar scope, size, complexity that are ongoing or have been completed within the last five (5) years. List the following information for each contract: Company or entity name and address Description of services performed Name, telephone number and email address of responsible individuals who have first-hand knowledge of performance relative to similar type of services (Reference Point of Contact) Dates of contract performance Contract Type (e.g. fixed-price, cost reimbursable) and total contract value 2. Questions to this RFQ shall be submitted via electronic mail to Contract Specialist Tyrone Whalen via email to tyrone.whalen@va.gov. Questions shall be accepted until 5:00pm EST 12 August 2024. The Government is not obligated to respond to any questions received after that date and time. All questions received within the designated timeframe that the Government considers relevant, will be answered. Responses to questions will be provided as an amendment to the RFQ that will be made available to all potential Offerors. Telephonic inquiries are not acceptable and will not receive a response. Provisions that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. The following provisions are incorporated into 52.212-1 as an addendum to this solicitation:
