- Special Notice 2 | DESCRIPTION The Department of Veterans Affairs, Network Contracting Office 12, hereby provides notice of its Intent to award a Sole Source, Firm Fixed Price contract to Axiom, Inc., a small business, located at: N16W23430 Stone Ridge Dr, Waukesha, WI 53188. The contract is expected to be awarded under the authority of 41 U.S.C. 3304(a)(1), as implemented by FAR 13.106-1(b) Soliciting from a Single Source. A Justification & Approval (J&A) for Single Source Awards for Other Than Full and Open Competition, (OTFOC) has been prepared as there is only one identified responsible source, and no other supplies will satisfy agency requirements. The total value of the award is $23,785.00 determined to be fair and reasonable by the Contracting Officer based on the current commercial price list available to the public. The anticipated award date is August 23, 2024, close of business. The Contractor will provide Custom Cabinetry Shelving Units used in conjunction with other pertinent items, Axiom, Inc., this end-product is to be utilized at Sterile Processing Dept (SPD). The Program Office is requesting a sole source for a custom-made storage cabinet shelving units because it will greatly help (SPD) Sterile Processing Department in efficiently storing sterile instruments and efficiently organize different surgical instruments. Better organization will significantly reduce the amount of misplaced or lost instruments which is critical to patient care. In addition, the vendor is located locally and can come out and custom-fit these SPS medical cabinet shelves on a short notice which saves time and money for the VA in terms of providing the lodging and air travel expenses for other non-local vendors to visit this VAMC. Also, this local contractor Axiom Inc. will furnish all supplies, equipment and services required for delivery, installation and any testing needed of the supplies and equipment. The main unique feature of each cabinet to be configurated with drawers and rails to fit each surgical specialty instruments precisely such as Robotic, General Surgery, Orthopedic, Urology, ENT, etc. These heavy-duty cabinets will allow for multiple railing to hang specialized instruments and to place stock in draws separating the various items for sanitation. Per end-user, these customized Axiom cabinets shelving have been installed in several sites within the VA VISN 12 such as Hines VA and Milwaukee VA. However, interested parties may identify their interest and capability to respond to this requirement within the deadline of this notice. A determination not to compete this requirement, based upon responses to this notice, is solely within the discretion of the Government. Information received will normally be considered solely for determining whether to conduct a competitive procurement. The Department of Veterans Affairs will NOT be responsible for any costs incurred by interested parties in responding to this Notice of Intent. Only written responses will be considered. All responses from responsible sources will be fully considered. As a result of analyzing responses to this Notice of Intent, the Government shall determine if a solicitation will be issued. Any prospective Contractor must be registered in the System for Award Management (SAM) to be eligible for award and must submit a current Authorized Distributor letter from the manufacturer (If applicable). All Service-Disabled Veteran Owned and Veteran Owned Small Businesses must submit a self- certifying statement that their company meets all criteria established IAW 13 CFR 121.406(b)(1)(i) through (b)(1)(iii) thus demonstrating compliance with the Non-Manufacturer Rule.
