- MEMORANDUM FOR SIMX, INC. FROM:��� 763 Enterprise Sourcing Squadron (763 ESS) 510 POW/MIA Drive, Bldg. P-40, Suite E1004 Scott AFB, IL 62225 SUBJECT: Simplified Acquisition Proposal Request (SAPR), FA445224R0018, Single Source - Brand Name Only (BNO) SimX Virtual Reality (VR) This is a SAPR for the subject requirement to be awarded pursuant to Federal Acquisition Regulation (FAR) FAR Part 13 ordering procedures, FAR Part 12 procedures, and the specific requirements outlined in this request with attachments. Headquarters Air Mobility Command (AMC) Surgeon Generals Medical Modernization Branch (SGR) requires the purchase of SimX VR Training Kits. It is the intent of the 763d Enterprise Sourcing Squadron to award a single source-brand name only, firm-fixed price (FFP) purchase order to SimX, Inc. located at 548 Market Street #87927, San Francisco, CA 94104, CAGE 79S56. The awardee shall deliver the BNO SimX VR products in accordance with the delivery dates outlined in Attachment 1. In response to the SAPR, the contractor shall complete and return the following:� � � � � � � � � � � � � � � � � � � � � � � � � � � � � � - Information and Pricing Worksheet (Attachment 1), or a submitted proposal in offeror format as long as it, at a minimum, depicts unit prices, extended prices, and a total (rounded to the nearest dollar), provides applicable specifications, and submits a response providing the information being requested in paragraphs 1 � 11� � � � � � � � � � � � - Provisions and Clauses (Attachment 2), the contractor shall complete the fill-in provisions and clauses outlined in Attachment 2.� � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � �- Redacted Single Source Justification (Attachment 3) (Will be provided/posted to SAM.gov by the Contracting Officer and/or Contract Specialist)� � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � � - The contractor shall include a cover letter stating a proposal acceptance period of at least 30 days.� Proposals offering less than 30 days will be deemed unacceptable.� The cover letter shall be submitted on company letter head that includes the company name, address, and a specific point of contact (POC).� � � � � � � � � � � � � � � � � � � � � � � � � � � � By submission of a proposal, the contractor accedes to all SAPR requirements including terms, conditions, and representations. The contractor�s proposal shall be valid for at least 30 days after proposal submission. Award will be made in accordance with the simplified acquisition procedures, FAR Part 13, the procedures specified in the SAPR, and on the basis of the single source-brand name, firm fixed price proposal that is technically acceptable. An order may be awarded to the offeror who is deemed responsible IAW FAR 9.1, as supplemented, whose proposal conforms to the SAPR's requirements (to include all stated terms, conditions, representations, certifications, and all other information required by the SAPR instructions) and is judged to represent a technically acceptable proposal. Questions and Answers / Assumptions and Exceptions:� Contractors are cautioned that all communications are to be channeled through the Contracting Specialist (CS) and Contracting Officer (CO).� Any questions whether technical or contractual in nature shall be submitted in writing.� Questions and Answers may be submitted in writing to the CS Caydn Schuchman at caydn.schuchman@us.af.mil AND CO Tonya Hughes at tonya.hughes.2@us.af.mil by 3:00 PM Central Standard Time (CST) on 5 August 2024 to the SAPR.� No further questions will be addressed after this date.� Contractors shall address any assumptions and proposed exceptions to the CO in their proposal.� This SAPR is not authorization to provide the products or begin performance, and in no way obligates the Government for any costs incurred by the offeror associated with developing a proposal. The Government reserves the right not to award a contract in response to this SAPR. Prior to commencement of any activities associated with delivery or performance of this requirement, the Government will issue a written directive or contractual document signed by the Contracting Officer with appropriate consideration established. Use of Non-Government Advisors. Offerors are advised that technical data submitted to the Government in response to this SAPR may be released to a non-Government advisor for proposal review and analysis. All non-Government advisors will have signed non-disclosure statements. All company information and pricing will be redacted from the proposal prior to the non-Government advisor providing their review and analysis. Offerors shall complete paragraph 10 in Attachment 1 of the SAPR by responding to paragraph (a) or (b). Proposals shall be submitted via Sam.gov or via email to Contract Specialist Mr. Caydn Schuchman, caydn.schuchman@us.af.mil and Contracting Officer, Ms. Tonya Hughes, tonya.hughes.2@us.af.mil, no later than 3:00 PM Central Standard Time (CST) on 6 August 2024. Any proposals received after the SAPR closing date and time will be considered �late� and may not be considered for award.
