36C25024Q0880 SPECIAL NOTICE NOTICE OF INTENT TO SOLE SOURCE LUMENIS DIGITAL DUET LASER The Department of Veterans Affairs, The Dayton VA Medical Center, 4100 W Third St, Dayton, Ohio 45428 intends to solicit and negotiate with only one source under the authority of FAR 6.302-1(a)(2), Only one responsible source and no other supplies or services will satisfy agency requirements. The VA intends to solicit and negotiate with (OEM) Lumenis , 2077 Gateway Place Suite 300 San Jose, CA 95110 only Authorized Distributor: Service-Disabled Veteran Owned Small Business (SDVOSB), Melling LLC, 658 South Columbus Street, Alexandria, VA 22314. As (OEM) Lumenis does not distribute directly to the government and all sales go thru their only Authorized Distributor: SDVOSB, Melling LLC. The Dayton VA Medical Center s Eye Clinic is in need of replacing its current Lumenis Digital Duet Laser that has reached end of life. The Digital Duet Laser is needed to perform Selective Laser Trabeculoplasty (SLT) and YAG (yttrium-aluminum-garnet) capsulotomy on Veterans that will need it for possibly blinding eye diseases like glaucoma treatment or removing of secondary membrane after cataract surgery. Both procedures are crucial to prevent visual disturbances and in some cases blindness. The Digital Duet is also the 1st and only Digital platform, allowing to take images and videos of the treatment, generate digital treatment reports, and connect to the local Electronic Medical Record (EMR). The Digital Duet Laser is an all-in-one anterior segment laser platform, that allows the physician to treat all the anterior segment laser procedures of SLT, Iridotomy, Capsulotomy and Posterior Membranectomy, including Vitreous Opacities Illumination). If the Eye Clinic does not have this Lumenis Digital Duet Laser, then our Veterans will have to seek care in the community which will lead to an increase in cost for the Department of Veterans Affairs. Number: 1 552-24-2-121-0127 The Government believes that only this business can satisfy the agency requirements and intends to negotiate with only one source under the authority of FAR 6.302-1(a)(2), Only one responsible source and no other supplies or services will satisfy agency requirements. Other persons may identify their interest and capability to respond to this requirement by Friday, August 2, 2024, by 5:00PM EST. Minimum Salient Characteristic(s) Digital Duet is an all-in-one anterior segment laser platform, that includes both SLT and YAG. Therefore, allowing a physician to treat all the anterior segment laser procedures of SLT, Iridotomy, Capsulotomy and Posterior Membranectomy (including vitreous opacities illumination). The Digital Duet is also the 1st and only Digital platform, allowing to take images and videos of the treatment, generate digital treatment reports, and connect to the local EMR. 1. Upgradable: Only SLT/YAG system that is upgradable at any time to include green photocoagulator (Smart532) and sub-threshold capabilities (SmartPulse ). 2. Two legs table- ADA compliant: ADA compliant, therefore allows true wheelchair accessibility with convenient control features. 3. Imaging and video: an integrated high definition camera, allowing to capture images and videos, before, during, and after the treatment. 4. Treatment reports: Automatic generation of treatment report, which includes all patient information and laser settings, and also ability to add predefined comments. Reports can also include images you captured during the treatment. 5. Connectivity: System has USB ports, Wi-Fi connection, and LAN connection. You can choose to save your data to a USB drive, or to a folder on your local network. 6. EMR integration: the Digital Duet integrates to your EMR. Images and reports can be sent directly to the patient file on the EMR. 7. Superior illumination and optics with Smart-V : a. On-axis and off-axis illumination in SLT and YAG treatments. b. Red reflex free (with Smart-V): two converging light beams that allow superior visibility of the vitreous without red reflex. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: This device will need to be brand name due to it solely being made by Lumenis. The Digital Duet Laser is an all-in-one anterior segment laser platform, that includes both Selective Laser Trabeculoplasty (SLT) and YAG (yttrium-aluminum-garnet) laser. This allows the physician to treat all the anterior segment laser procedures of SLT, Iridotomy, Capsulotomy and Posterior Membranectomy, including Vitreous Opacities Illumination). Description of market research conducted and results or statement why it was not conducted: Per End User: Personal knowledge of Lumenis made laser devices was applied in performing market research of this item. Contacted knowledgeable individuals in the industry regarding the laser system. Reviewed catalogs and on-line product literature. Per Contracting: The following market research techniques were used but are not limited to: Market research was conducted via VetCert, DSBS, GSA, and the Internet. The VetCert search using NAICS 334510 netted 316 SD/VOSBs. The DSBS search using the same NAICS netted 1,819 vendors. The GSA Advantage search using LOCAL STOCK NUMBER: GAS910000DIG netted 0 vendors.
