- 36C25024Q0817 Special Notice Intent to Sole Source GOLVO Mobile Floor Lifts The Department of Veterans Affairs, The Chalmers P. Wylie Ambulatory Care Center, 420 North James Road, Columbus, Ohio 43219 intends to solicit and negotiate with only one source under the authority of FAR 6.302-1(a)(2), Only one responsible source and no other supplies or services will satisfy agency requirements. The VA intends to solicit and negotiate with the OEM, Hill-Rom, Inc. at 1069 State Road 46 E, Batesville, Indiana 47006-7520 and their three Service-Disabled Veteran-Owned Small Business (SDVOSB) authorized distributors: SDVOSB, Electra-Med Corporation at 5332 Hill 23 Drive, Flint, Michigan 48507; SDVOSB, Geo-Med, LLC at 1525 International Parkway, Ste. 3071, Lake Mary, Florida 32746 and SDVOSB, The Janz Corporation at 275 Outerbelt St., Suite F, Columbus, OH 43213. The Chalmers P. Wylie Ambulatory Care Center is in need of new and replacement of near-end-of-life mobile floor lifts. The mobile floor lifts will have the ability to extract patients out of cars/vehicles, recover patients from the floor, transfer patients from chair to exam table and back, boosting, proning, ambulation capability and act as a ceiling lift alternative when needed. The lift is to be powered with a lifting capacity of 440 pounds with a turning radium of 52 inches. The lift is compatible with current Guldmann and ARJO slings. Contract Title Replacement of near end-of-life mobile floor lifts and a NEW request for mobile floor lift for Chalmers P. Wylie Ambulatory Care Center. The mobile floor lifts will have the ability to extract patients out of cars/vehicles, recover patients from the floor, transfer patients from chair to exam table and back, boosting, proning, ambulation capability and act as a ceiling lift alternative when needed. The lift is to be powered with a lifting capacity of 440 pounds with a turning radium of 52 inches. 3. Background The National VISN SPHM Directive 1611 states that all staff are to use SPHM equipment to reduce the potential for risk of staff/veteran injury. Currently, the main facility has 2 operable working mobile floor lifts for multiple areas where patient care is provided. 2 mobile floor lifts were decommissioned because of end of life. 1 Bariatric mobile lift is located at the front entrance because it is the only mobile lift that will safely maneuver over the Braille bumps. We are requesting 2 Liko Golvo 9000 Low base mobile floor lifts to replace 2 that were decommissioned. We are also requesting 1 NEW Liko Golvo 9000 Low base mobile floor lift to be placed at the back entrance of the main facility so that it is easily accessible. The Golvo mobile floor lifts will be utilized for transferring patients between devices (e.g., within the room), floor lifting, horizontal lifting, supporting patient limbs, ambulating patient, transferring patients to/from vehicles. Lift has the ability to use ambulation harness and has ambulation capability. Ability to use ambulation arm to help maneuver lift turning. Intended for use in all areas that provides patient care and can act as a ceiling lift when needed. The Golvo has parallel base-width adjustment legs that makes it easier to lift patients to/from chairs, beds and the floor. Retractable armrests help keep patients and caregivers stable, safe and secure. GOLVO 9000 Low base gives extra clearance in tight spaces (e.g., beneath our Stryker stretchers). Vertical lifting motion with flexible lift strap is ideal for horizontal lifting, ambulation and lifting from the floor. IT has a parking panel for hanger bar when not in use. Plugs into the wall to recharge the battery. No other mobile floor lifts within the facility have the capabilities that the Golvo 9000 mobile floor lift offers. The GOLVO 9000 parallel base-width adjustment legs is the only mobile floor-based lift with parallel base-width adjustment legs on the market. Other SPHM mobile floor lifts open in a V shape. The lift is compatible with current Guldmann and ARJO slings. This is the only mobile floor lift that has retractable ambulation arms which has the ability to use an ambulation harness and has ambulation capability - This can be used in physical therapy if needed. Also, the ambulation arms can be used to assist maneuvering the lift in a 360-degree turn. The Liko Golvo 9000 has low caster wheels which are small enough to fit EASILY under the Stryker Stretcher that is being used throughout the facility NO other lift within the facility has this capability. 4. Scope. The contractor shall provide all labor, supervision and all other resources required to deliver the requirements stated herein, except as may otherwise be specified in this statement of work. The portable mobile floor lift is to be low base, powered with working ambulation arms. The lift must have a working capacity of 440 pounds and a turning radius of 52 inches. The lift must have a quick-release hook system that makes changing the sling bar quick and easy. It must have the ability to operate by hand control or control panel. Also, it must have a vertical lifting motion with flexible lift strap. 5. Specific Tasks. The Golvo lift will be used for car/vehicle extraction, floor recovery, ambulation, transfers, boosting, repositioning, and act as a ceiling lift alternative. It will be used with a reusable sling and is compatible with current Guldmann and ARJO slings. 5.1 Task 1 - Enterprise Management Controls. 5.1.1 Subtask 1 - Integration Management Control Planning. Provide the technical and functional activities at the required level for integration of all tasks specified within this SOW. Include productivity and management methods such as quality assurance, progress/status reporting and program reviews. Provide the centralized administrative, clerical, documentation and related functions. 5.1.2 Subtask 2 - Contract Management. Prepare a Contract Management Plan describing the technical approach, organizational resources and management controls to be employed to meet the cost, performance and schedule requirements throughout Contract execution. Provide a monthly status report monitoring the quality assurance, progress/status reporting and program reviews applied to this contract. 6. Performance Monitoring Lift must come with a wall plug and a second battery and a wall charger. Patient lift must hold at least 440 pounds and be intended to lift a dependent patient from a floor or vehicle to any chair or exam table and back. Patient lift must operate using a flexible belt with a 2-point hanger bar that can reach into the vehicle. Lift must allow use of other manufacturers loop slings, including Guldmann, ARJO, and EZ-Way slings. Lifting and lift base widening/narrowing must be motorized. Lift must have low base option which gives extra clearance in tight spaces (e.g., beneath current Stryker stretchers). Lift must lift low enough to pick up a patient off the floor in a universal high back sling and high enough to elevate patient s hips at least 28 inches in the longest sling loops. (Lifting range must be at least 50 inches.) Lift must fit through a 29 door at its narrowest setting. Lift must have a universal sling bar 450 for ease of getting a patient moved out of a Car/vehicle, floor recovery, boosting and transfers. Lift must come with a reusable, friction-reducing sling designed for car extractions. Lift must be warranted for at least 1 year. Longer warranty terms are preferable. Lift must include training for BioMed and frontline clinical staff. 7. Security Requirements The C&A requirements do not apply, and that a Security Accreditation Package is not required. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI) NA 9. Other Pertinent Information or Special Considerations Training: Contractor shall provide certified in-service training using sling for facility onsite in person for staff utilizing new Bariatric Portable Powered lift for transfers, floor recovery and car/vehicle extraction. Manuals: Contractor shall provide written electronic copies of the user manuals and repair manuals for each model of motor/hoist replaced under this order or contract at no additional cost to the government. NA Packaging, Packing and Shipping Instructions. NA Inspection and Acceptance Criteria. NA 10. Risk Control Safety recommendations Cleaning and disinfecting procedures for Liko Mobile lifts. These instructions do not replace the facility s own cleaning and disinfection policies. Wear protective equipment according to manufacturer s instruction and per facility protocol throughout the cleaning operations, such as: rubber gloves, goggles, apron, face mask and shoe covers. Unplug mains (AC power source) before cleaning and disinfection. Never clean the lift by pouring water on it, steam cleaning it, or by using a high-pressure jet. Refer to the recommendations made by the cleaning and disinfecting product manufacturer. Equipment: Protective equipment (such as: rubber gloves, goggles, apron, face mask and shoe covers) as recommended by the facility protocol and manufacturer s instructions. Clean buckets Cloths for washing and drying. Soft brush Warm water To find Cleaning / Disinfectants compatible or not compatible for use on Liko s products, follow the Application of commonly used Cleaning / Disinfectants on Liko products in this document. Cleaning instructions Unplug mains (AC power source) before cleaning and disinfection. Clean the lift with a cloth moistened with warm water and a neutral cleaning agent approved by your organization. A soft brush may be used to remove stains and resistant dirt. Wipe down the entire lift starting from the top down. Do not use a cloth that is dripping wet. To have access to all areas run the lift into the highest and lowest positions and extend the base width adjustment entirely out. Remove the Battery to have access behind the battery. Lower the sling bar to get access to the whole lift strap. Fully extend the lift strap by using the emergency lowering. After cleaning the lift strap, and before you raise the sling bar, make sure the lift strap has dried. After the emergency lowering device is used, the lifting level needs to be restored. Pay special attention to the following areas: Lift strap Sling bar (different designs) Armrest Mechanical emergency lowering Hand control Handles Control box Emergency stop Battery Wheels Disinfection Instructions For the use of suitable disinfectants see Application of commonly used Cleaning / Disinfectants on Liko products in this document. Use the choice of disinfectant according to the manufacturer s instructions and repeat the work step as in Cleaning instructions"". Remove traces of disinfectant after disinfection. Wipe off the lift with a cloth moistened with clean water starting from the top and working down. The cloth shall not be so damp that it drips. The lift may not be cleaned with CSI or equivalent. The hand control may not be cleaned with Viraguard or equivalent. The control box may not be cleaned with Anioxy Spray or equivalent. The lift strap may not be cleaned with Oxivir Tb, Dispatch, Chlor-Clean, Dismozon Pur or equivalent. 11. Place of Performance Chalmers P. Description of market research conducted and results or statement why it was not conducted: Per End User: Multiple methods were utilized to review available market options. A vendor fair was provided to staff onsite. This writer conducted an online search of available product options. This writer spoke with multiple VA s across the country. The SPHM Program Manager at the Cincinnati VA states that their facility utilizes the Golvo 9000 for multiple uses to include car extraction/full fall rescue, boosting, repositioning and transfers. They highly recommend this product and have multiple Golvo 9000 mobile floor lifts onsite. VHA SPHM Program Manager for the Lexington VA Medical Center Lexington, KY recently completed market research to purchase the Liko Golvo 9000. It was purchased because it is user friendly, easy to understand and implement procedures when in use by staff. Discussing with other SPHM PM s, VHA Medical Centers nationally are using this equipment with great success. The Golvo 9000 was demoed by the main facility staff in a staff/vendor simulation demo. Staff agreed that the unit is user friendly, easy to understand and has a maximum weight load of 440 pounds and has ambulation capability. This writer and staff agreed that this lift is portable, and floor based for areas outside of the limited spaces covered by ceiling lifts. It also fits easily under the current Stryker Stretchers within the main facility. Per Contracting: The results of the market research showed that OEM, Hill-Rom limits the sale and service of Golvo Mobile Floor Lifts as they utilize authorized distributors to further their products. The following market research techniques were used but not limited to: VetCert, DSBS, GSA and Internet. The VetCert search using NAICS 339112 netted 658 SD/VOSBs. The DSBS search using the same NAICS netted 3,811 Vendors; added keyword GOLVO and netted 0 Vendors. The GSA search using the same NAICS netted 0 Vendors. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b).
