- The purpose of this amendment 0001 to RFQ 36C25924Q0537 is to post vendor questions and answers, post updated SOW to clarify requirement, post additional supporting attachments and provide for a quoted submission deadline extension. Quote Deadline Quotes are to be provided to natasha.holland@va.gov no later than 4:00pm MT, July 08, 2024. Quotes shall not be hand carried. Faxed quotes shall not be accepted. Emailed quotes are the only method of submitted quotes (4MB email limit). The Government reserves the right to make award solely on initial Quotes received. Offerors bear the burden of ensuring that all portions of the offer (and any authorized amendments) reach the designated office before the deadline specified in the solicitation. This RFQ shall be completed in its entirety, and signed and dated, failure shall not be considered for award. Questions and Answers Q1. Does it have to be a pendant mount? A1. No, pendant mount is an example of an acceptable mounting method. Threaded rod solutions will not be accepted. Q2. Does the vendor have to provide stamped drawing of the mounting plan? A2. Yes, per VA master specification attachment. Note any additional requirements outlined in SOW must be met due to structure integrity needs. Please be sure to address each requirement in the SOW in responses. Q3. Finished ceiling heights? A3. Per ceiling drawings provided in ceiling heights attachment. Q4. What is the support structure? Is this I-beams, concrete, steel bar joist or another material? A4. Existing structure is concrete. Q5. Do you have any other rooms drawings because the ones provided do not contain any room numbers? A5. Additional drawing attached with room measurements and room number see floor plan attachment. Q6. Also, in a room 7 x6 an 1000lb system will not be effective there is no functional lift height nor width. We would recommend an 825lb system instead. Is this acceptable? A6. No, and 7 x6 is the H-track dimensions not the room size. Floor plan attached shows the room sizes. Q7. In radiology they are requesting a 1000lb motor, but from our experience this is a higher weight capacity than the table can support. Has this room been designed to support a 1000lb patient? A7. No, but it may be in the future. Design for 1,000 lbs. Q8. The SOW also mentions a curved traverse rail. No manufacturer makes a curved traverse rail. Can you confirm what is required here? A8. The curve is not necessary as long as the bed to bathroom transfer is achieved with a single lift of the patient. Q9. Will a lift with a single motor with lifting capabilities of over 1000lbs be considered versus a two motor 1000lb lift? A9. No. Q10. Will the door header be modified by the General contractor doing the remodel or will it be completed during the patient lift installation? A10. No, however, the door frames for bathrooms with the transition section for Bed to Bath transfer are being designed to accommodate and modification should not be necessary. Q11. Can you provide a current construction schedule showing when the lifts are to be installed in conjunction of the current construction project? A11. No, the awarded vendor will need to coordinate with the COR to obtain scheduling data after the award. The VA will accommodate as much as possible, but the VHIT, LLC will have the final decision on when the installation can occur. Q12. What is the anticipated award date for this contract? A12. The anticipated award date cannot be stated or guaranteed. Q13. Can you provide pictures or drawings that show the different types of decking the lifts will be mounted too? A13. Awarded vendor may be provided additional drawings during coordination meetings. The provided drawing of the reflected ceilings is the only drawing provided at this time. Q14.Will storage be provided by the gov t or will contractor need to provide? A14. No. Q15. Will power be provided for the new lifts? A15. Yes. Q16. Will a dumpster be provided? A16. No. Q17. Please confirm that none of the lift install will take place in a active hospital setting. A17. Confirmed Q18. Will annual weight tests be required, if so for how many years? A18. Only the initial certification test will be required on the contract. Q19. How many phases will be required for project? If multiple what is the breakdown? Upper structure, hanging rails and final hang traverse rail and weight test. Etc A19. Phases are not required. Coordinate installation after award. Q20. Can installation occur on weekends? A20. No Q21. Equipment delivery? Any special times and day? Will weekend work be allowed? A21. As of right now normal working hours is expected; however, these hours are subject to change and will not be guaranteed. Q22. Why was the first bid cancelled? A22. The quote was canceled due to changes to the SOW identified after RFQ closing. Q23. Can the VA provide a list of changes from the first bid? A23. No, old SOW is now obsolete. Please be sure that quote addresses all requirements outlined in updated SOW dated 06/24. Q24. What are the room numbers for JSN A1200? A24. Issue addressed on the revised SOW. Q25. A1200 is not listed on page one with the rest of the JSN numbers, but it is listed on page 4. What is the reason? A25. Issue addressed on the revised SOW. Q26. Do the Ceiling Lift vendors need to perform a PSI rating of the Concrete? If so, what is the preferred method used to calculate PSI rating? A26. Follow VA Master Specification for Certifications and Drawings Q27. What is the distance from the finished ceiling to the sub-structure (concrete) height? Floor to Floor height is about 12 6 with varying concrete floor thickness. A27. Additionally, drawings will be provided to the successful contractor. Q28. Is union labor or prevailing wages required for this installation? A28. Wage Determination has been provided. Q29. How many clinical training days are required for the staff? A29. Vendors should anticipate having to give detailed training to at least a dozen critical employees that need to know enough about the systems that they can teach safe operation to others. Additionally, they should anticipate having to do basic operation and safety training to approximately 80 personnel. These training may be spread out over the course of a two-week time scheduled during the activation time frame between August 2025 and December of 2025. Q30 Is biomedical training a requirement for this project? A30. Yes, see above. Q31. Are slings a requirement to be included on the RFQ? A31. Yes, minimum of 1 per lift total of 89 slings. Q32. Are scale attachments a requirement for this solicitation? If so, which rooms will require scales? A32. We will accept scale attachments if provided; however, they are not required per the scope of work. Q33. When is the expected timeline for award and installation? A33. Install between December 2024 and September 2025.
