- This is a combined synopsis/solicitation for brand name or equal commercial items prepared in accordance with (IAW) the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Government reserves the right to make no award from this solicitation. This is a request for quote (RFQ), and the solicitation number is 36C25024Q0776. The Government anticipates awarding a firm-fixed price Purchase Order resulting from this solicitation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circulars FAC 2024-03 (eff. 2/23/2024). The associated NAICS Code is 339112, Surgical and Medical Instrument Manufacturing. The Department of Veterans Affairs, Network Contracting Office 10, is soliciting quotes from all sources to supply the VA Northeast Ohio Healthcare System (VANEOHS) Optometry Section with a systematic upgrading of its core equipment throughout the Outpatient Clinics, to include slit lamps and tonometer s. Quotes are to be provided to DeJanee R. Evans, Contract Specialist, via email at dejanee.evans@va.gov, no later than July 3, 2024, by 5:00 PM EST. No telephone calls will be accepted. No submission received after the above-mentioned date and time will be accepted. STATEMENT OF WORK The VA Medical Center (VAMC), Cleveland, OH has a requirement for slit lamps and tonometer s. The Contractor is to provide slit lamps and tonometer s for the VANEOHS Optometry Section. DELIVERIES: Deliveries shall be made to: Cleveland Louis Stoke VAMC 10701 East Blvd Cleveland, OH, 44106 Background The VANEOHS Optometry Section request a systematic upgrading of its core equipment throughout the Outpatient Clinics, to include slit lamps and tonometer s. The current equipment is outdated, out of warranty, and with compromised function for delivering high-quality eye care for the VANEOHS Veterans. Core equipment is essential for delivering eye exams and has a significant effect upon our ability to accurately evaluate patients, provide proper patient positioning, determine glasses prescriptions, allow for optimized efficiency and ergonomics for our Optometry team. Scope. The requested equipment is listed below by section and include required specifications/features: Sixteen (16) Haag Streit BQ900 LED Slit Lamp (HS0900.0011LED) LED Illumination Sixteen (16) Haag Streit Goldmann Applanation Tonometer BQ 900 (HS09004.4) Fits the Haag-Streit slit lamps BQ 900 The VA Northeast Ohio Healthcare System is requesting the removal, purchase, delivery, installation, and configuration services for the patient care phoropters. The requested equipment shall be consistent with recent acquisitions of slit lamps and applanation tonometer s within the Medical Center. This equipment (or equivalent) is being requested for consistency and performance as mentioned above. The requested slit lamp equipment solution shall provide, but not be limited to, the following functionalities/Capabilities: LED illumination head Inclined Eyepiece with 20-degree upwards inclination to alleviate neck discomfort. Background illumination Eyepiece with double cross hair reticule Knob for beam splitter/camera on-off Aperture control knob Diffusor Imaging Module 910 Shutter-release Illumination control Beam splitter for slit lamp system that permits selective diversion of 50% of light to secondary slit lamp system attachment (observation tube). Observation tube (length ~ 150mm) with adjustable eyepiece for attachment to beam splitter of slit lamp system to permit observation by trainees. Stereo Variator ability to reduce angle from standard 13 degrees to 4.5 degrees. Unit model slit lamp that can be mounted on existing ophthalmic instrument stands (Reliance 7800-IC Floor Unit ophthalmic instrument stand). Slit image width of 0 to 14mm continuous. Slit image length of 1 to 14mm continuous. Illumination field apertures of 8, 5, 3, 2, 1 & 0.2mm. Slit image radial range +/- 90 degrees. Swiveling of the slit illumination relative to the microscope axis: +/- 90 degrees horizontally 0 to 20 degrees vertically Filters: Including blue, red-free (green) and grey (10%) Light source: LED with 400-750nm range of spectral illumination Microscope stereo angle of 13 degrees. Microscope magnification changer including 6.3x, 10x, 16x, 25x & 40x. Eyepiece Magnification: 12.5x Range of adjusting eyepieces: +7 diopters to -7 diopters. Interpupillary distance range: 52 to 78mm. The Haag-Streit 900 BQ Slit Lamp is standardized equipment in all currently VAPORHCS eye exam rooms. The requested tonometer equipment solution shall provide, but not be limited to, the following functionalities/Capabilities: Goldmann style applanation tonometer for use with Slit Lamp System above Measurement range 0 80mm Hg Measuring force Generated by leverage weight. Specific Tasks. The requested equipment solution shall provide the replacement of the above listed equipment in accordance with the listed salient characteristics provided under the scope to meet the requirement in full. The contractor is responsible for providing the delivery, removal, and installation of all equipment and coordinate the shipment/delivery to the locations listed above within sufficient time to allow installation in each exam room. All installation services shall be provided through the original manufacturer of the requested phoropters to various CBOC locations and shall be responsible for setup/installation by a fully trained/certified OEM representative(s), and finally, removal of the old units being replaced. The requested services shall be performed in accordance with the OEM s premium white glove delivery/install services. These services shall include, but may not be limited to, the following: A single point of contact for all project coordination Trained team from the OEM shall provide delivery and setup for all equipment and associated accessories. Delivery and setup service shall include delivery, uncrating, trash removal, product and accessory setup, and placement of units as needed. Manufacturer recommended operational test shall be performed and provide appropriate outcomes. 3.1 Task 1 - Enterprise Management Controls. 3.1.1 Subtask 1 - Integration Management Control Planning. Acquisition of purchased goods and services only. 3.1.2 Subtask 2 - Contract Management.
