Description

Provide all parts, labor, tools, materials, supplies, equipment, transportation, supervision for New Overhead Roll-up Garage Door and Installation necessary in support of the McClellan Memorial Veterans Hospital in Little Rock, Arkansas. This procurement is issued for Service-Disabled, Veteran-Owned, Small Business (SDVOSB).� SPECIAL INSTRUCTIONS 1. All quotes must be submitted directly to the Contracting Officer identified and listed in block 7a of the Standard Form (SF) 1449, by email to the following address: surella.tracy@va.gov. Quotes must be received no later than the date/time indicated in block 8 of the SF 1449. Any proposal received after the date and time referenced in block 8 may be considered late and may not be accepted. 2. All emails must reference �RFQ 36C25624Q1025, �Overhead Roll-Up Garage Door Replacement and Installation� in the subject line of the email. Failure to do so may result in mishandling or inadvertent deletion of emails. 3. QUESTIONS: All questions regarding this solicitation must be submitted to the Contracting Officer by email, no later than 10:00 AM, Thursday, June 27, 2024. 4. SUBMISSION REQUIREMENTS: All quotes must include the following, as a minimum. Failure to include any of the following with a quote may result in that quote being determined non-responsive. Any quote determined non-responsive will NOT be evaluated for award. 5. Interested parties must be registered in Veteran Small Business Certification (VetCert) at time of quote submission and award. a. Contractor�s business name (as registered in SAM) b. Contractor�s address (as registered in SAM) c. Contractor�s Unique Entity ID number (as registered in SAM) d. Point of Contact (POC) Name e. POC phone number f. POC email address g. Completed Price Schedule (Section B.1) h. Completed Contract Administration Data (Section B.3) i. Completed solicitation provision 52.204-24 (Section E.1) j. Completed solicitation provision 52.209-7 (Section E.3) k. Completed solicitation provision 52.212-3 (Section E.8). Complete and return only paragraph (b)(2) of this provision if your annual representations and certifications in SAM, have been entered or updated in the last 12 months, are current, accurate, complete, and applicable to this solicitation (including the business size standard(s) applicable to the NAICS code(s) referenced for this solicitation), at the time this offer is submitted.
