- This is a Sources Sought /Request for Information only. This notice is issued solely for information and planning purposes; it does not constitute a Request for Quote (RFQ)/Invitation for Bid (IFB)/Request for Proposal (RFP) or a promise to issue an RFQ, IFB or RFP in the future. This notice does not commit the U.S. Government to contract for any supply or service. The U.S. Government will not pay for any information or administrative costs incurred in response to this notice.� The Centers for Disease Control and Prevention (CDC) is looking to enter into a Blanket Purchase Agreement (BPA) with a minimum of two (2) and no more than five (5) small businesses to provide CO2 in the form of dry ice, solid CO2, bulk liquid CO2 tank refills and leased cylinders of CO2 with delivery to CDC Lawrenceville and Roybal campuses in Georgia. This BPA will have a five year ordering period. The results of any market research will contribute to determining the method of procurement. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 325120 � Industrial Gas Manufacturing. This type of requirement has previously been set aside for small businesses, but the U.S. Government will not be able to set aside this requirement if 2 or more small businesses do not respond with information to support the set-aside. We encourage all small businesses, in all socioeconomic categories (including, 8(a) Business Development Program, Small Disadvantage Business, Historically Underutilized Business Zone, Service-Disabled Veteran-Owned, Women-Owned Small Business concerns), to identify their capabilities in meeting the requirement at a fair market price. Please review attached statement of work and pricing schedule for requirement specifics. If your organization has the potential capacity to perform all of these services, please submit the following to the Point of Contacts (POC) information listed below: - Name & Address of your Firm - Point of Contact (name/phone/email) - Company website - Provide firm�s UEI and Cage Code. - Business Size, to include designation as HUBZone, 8(a), Service Disabled Veteran Owned Small Business, Small Disadvantaged Business, Woman-Owned Business, and large�business. The Government must be able to verify SBA certification of Hubzone, 8(a) and SDB�via the System for Award Management (SAM) [https://www.sam.gov/]. - Capability statement addressing the particulars of this effort, with appropriate documentation� supporting claims of organizational and staff capability.
