This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. 1.a. Project Title: Adult Male Full Body X-Ray/CT and MRI Phantom Model 1.b. Description: The Northern Arizona VA Healthcare System is seeking a contractor to provide an Adult Male Full Body X-Ray/CT and MRI Phantom Model with muscles. 1.c. Project Location: The Northern Arizona VA Healthcare System, located in Prescott, AZ. 1.d. Solicitation Number: Request for Quotes (RFQ) - 36C26224Q1278. 1.e. Set Aside: Total SDVOSB Set-Aside 1.f. Applicable NAICS code: 339112 Surgical and Medical Instrument Manufacturing 1.g. Small Business Size Standard: 1000 employees 1.h. Type of Contract: Firm Fixed Price 2. Scope. The Veteran s Integrated Service Network 22 is seeking a contractor to provide a True Phantom Solutions Adult Male Full Body X-Ray/CT and MRI Phantom Model with muscles or equivalent having the following salient characteristics, adhering to the S02 36C26224Q1278 attachment and provisions. Brand name Information: Manufacturer: True Phantom Solutions Nomenclature: Adult Full Human Body Phantom (With Muscles) for X-Ray CT & MRI Model Number: FB-A02 (Any other information): Equal to product Information: Salient Characteristics The model of this phantom is based on an adult male body and must weighs about 70 kg (154 lb) and must measures 175 cm (69 in) in height. Model must be XR, CT & MRI compatible which is valuable for training purposes across all modalities. Upon request, the phantom must be customizable with different pathologies such as lesions, tumors, infections, and abnormalities. The Model Anatomy Must Have: Full Human Body (10 Parts) Realistic Body Tissue-Mimicking Material Rotatable Shoulders (360° Around and 180° Sideways) Movable Hip, Knee, and Elbow Joints Detachable Head, Torso, and Limbs Adult Head Skull Brain Brain Tissue Grey and White Matter Air Cavities Frontal Sinus with Air Gaps Alignment Markings Adult Torso Spine Ribcage Shoulders Clavicles Pelvis Torso Organs Trachea Heart Lungs Diaphragm Liver Gallbladder Stomach Kidneys Spleen Pancreas Large and Small Intestines Bladder Prostate Adult Arms Humerus Elbow Joints Radius Ulna Wrist Fingers Adult Legs Femur Knee Joints Tibia Fibula Foot Toes Transparent or Skin-Mimicking Appearance Must have a hard carry case for storage. Soft tissue and organs must be made of: composition of urethane-based soft resin. Synthetic bones must be made of: Ceramic-reinforced epoxy-based composite material. Must have at least a 2-year warranty and unlimited customer service. 3.
