Description
The Department of Veterans Affairs Network Contracting Office (NCO) 19, Rocky Mountain Acquisition Center (RMAC) is seeking information and sources that can provide Coagulation testing for Veterans Integrated Services Network (VISN) 19; in the following Veterans Affairs Health Care Systems. VA Montana Health Care System: 3687 Veterans Dr., Fort Harrison, MT 59636 VA Billings Healthcare Center: 1766 Majestic Lane, Billings, Montana 59102 VA Cheyenne Medical Center: 2360 East Pershing Blvd., Cheyenne WY 82001 VA Northern Colorado, Loveland VA Outpatient Clinic: 4875 Byrd Dr., Loveland, CO 80538 VA Rocky Mountain Regional Medical Center: 1700 N. Wheeling St., Aurora, CO 80045 VA Grand Junction Health Care System: 2121 North Avenue, Grand Junction, CO 81501 VA Eastern Oklahoma Health Care System: 1011 Honor Heights Drive, Muskogee, OK 74401 VA Ernest Childers Clinic, 8921 S. Mingo Rd., Tulsa, OK 74133 VA Oklahoma City Health Care System : 921 Northeast 13th Street, Oklahoma City, OK 73104 VA Lawton/Fort Sill Outpatient Clinic: 4303 Pitman Street, Fort Sill, OK 73503 VA Salt Lake City Health Care System: 500 Foothill Drive, Salt Lake City, UT 84148 VA Sheridan Medical Center: 1898 Fort Road, Sheridan, WY 82801 The contractor shall provide all equipment, reagents, standards, controls, supplies, consumables/disposable items, parts, accessories, and any other items required for the proper operation of its Contractor owned coagulation equipment necessary for the generation of a patient Coagulation Cost Per Reportable Result (CPRR). A services component will also be provided for maintenance, repairs, upgrades, trainings, and quality control. Equipment supplies, and services shall be required for a base year period with four one-year options for renewal. This requirement is for VISN 19 Pathology and Laboratory facilities listed above. The NAICS code is 334516 Analytical Laboratory Instrument Manufacturing and Small Business Administration Size Standard is 1,000 employees.
