- ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS Provisions that are incorporated by reference by FAR 52.252-1, have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. The following provisions are incorporated into 52.212-1 as an addendum to this solicitation: Submission of Quotations: Quotes shall be submitted by the due date and time provided on the SF1449 block 8, CST. Offers shall be emailed to Yvette Kelly:� yvette.kelly@us.af.mil and Nelson Kraemer: nelson.kraemer@us.af.mil� �� One (1) electronic copy shall be emailed as follows: Tab A � Quotation Form (Standard Form 1449) � original signature and date; Contract Administration Data; UEI Number; Acknowledgement of Solicitation Amendments Tab B � PRICE PROPOSAL: Pricing shall be submitted using the attached Pricing work sheet.� Fill the unit price and company name.� All other amounts will auto fill. Return the pricing sheet with your quote.� Also, insert the same exact unit prices and amounts�from the pricing worksheet into page 3 of 43 Standard Form 1449 Solicitation.� Return both the completed pricing worksheet and solicitation with your quote.� These numbers will be the same, however, the government will use the attached pricing worksheet to determine your price as fair and reasonable for award purpose.� No other variation of pricing will be acceptable. Tab C - Completed copy of representations and certifications at FAR 52.212-3. Tab D - TECHNICAL APPROACH: The technical approach shall address each factor in 52.212- 2 and clearly identify which factor is being addressed.� Technical Competence and past performance must be included in your approach plan describing your procedure/approach to accomplish the service. Do not only copy and paste from the peformance work statement (PWS) and solicitation. Provide your own company's ideas and solution to the issue. Technical approach shall be no longer than 3 pages, using 12 size font and Times New Roman font style. Tab E � PAST PERFORMANCE PROPOSAL: Quotations shall provide their past performance. The past performance shall include recent and relevant contracts for the same or similar items. Each identified contract shall include the following information: contract numbers, points of contact of Government personnel who accepted/received the products/services, phone numbers and email addresses for those Government personnel, a description of the requirement and an explanation of how it relates to the current requirement. Recent contracts are those contracts with ordering periods or deliveries occurring, at least in part, within the last three (3) years. Relevance, for the purposes of this procurement, is defined as substantially similar to the current requirement in terms of effort, complexity and contract type. Offerors shall provide no more than three (3) examples of past performance. The past performance information shall be no longer than 5 pages, using 12 size font and Times New Roman font style. The request for quotes due date has been extended to 3 June 2024, 3PM CDT. Changes due to amendment1:�� 1. See addendum to FAR 52.212-1 above 2. PWS: Military Sports Bike Rider Course (MSRC) no longer needed and the service must be provided within 25 miles of Tyndall AFB, Florida. See new attached PWS 22 May 2024 3.�The request for quotes due date has been extended to 3 June 2024, 3PM CDT. 4.� Pricing worksheet has been added and attached. 3rd Question:��How frequently would the class be taken?"" I ask to understand how to price this solicitation. Answer:��One or two classes per month has been the norm in previous years. 2nd Question:� Will the training to be held at Tyndall AFB, a commercial lot, or a lot that is already provided? Any information would be greatly appreciated Answer:��The training facility will need to be held at the contractor�s site. We do not have a motorcycle range on base. My email address has been updated to Yvette.kelly@us.af.mil Below is a question and answer to solicitation #FA481924Q0010: 1st Question:��Can you give me an idea of roughly how many people would be in the safety training classes on a regular basis?� Answer:��Typically between 4 to 12 students per class A updated Soliction #FA481924Q0010 will be added to the attachments. The contractor shall provide all managment, tools, supplies, equipment, and labor necessary to provide motorcycle safety training to military personnel assigned to Tyndall AFB.� Other Department of Defense (DoD) civilian personnel, military retirees, military dependents and civilians may attend motorcycle operator training on a space-available basis at no additional cost to the Government.� The list of personnel trained will be provided by the contractor to the 325 FW Occupational Safety Office (325 FW/SEG) within one of training completion. 3 Attachments: Solicitation #FA481924Q0010 WD2015-4559 RV 24 dated 26 December 2023 Performance Work Statement (PWS) dated 26 March 2024
