THIS REQUEST FOR INFORMATION (RFI) 36C26224Q1141 IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a source sought/RFI for market research only. The purpose of this source sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 334118 (size standard is 1,000 Employees). Responses to this source sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this source sought, a solicitation announcement may be published. Responses to this source sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office, is seeking sources that can provide Handheld BCMA Barcode Scanners that at a minimum meets the following salient characteristics in the Statement of Work for the VA PHOENIX HEALTHCARE SYSTEM: Item Number Description/Part Number ( EQUAL TO ITEMS SATISFYING THE SALIENT CHARACTERISTICS LISTED IN THE MTR WILL BE ACCEPTED FOR ANY ITEM LISTED BELOW WILL BE ACCEPTED; PLEASE SEE MTR FOR RANGE OF ACCEPTABLE RANGES OF MEASUREMENTS FOR ITEMS LISTED BELOW) Quantity Unit of Measure Unit Price Amount 0001 1952HCF-CG3 1 SEWP CLIN A035658 Enhanced Honeywell-based 1952HHD Cordless BCMA Firmware Scanner Kit PART# 1952HCF-CG3 OR EQUAL TO Includes BCMA Firmware, setup card, FIPS 140-2, USB, 3 year Comprehensive warranty (TCG1902-1LC3) with TCG EWP Warranty Protection. 25 EA     TOTAL   Include shipping cost on the items price. Vendor Requirements: Period of Performance: 60 days after receipt of order (ARO). Vendor shall quote all items under this requirement. Vendor shall meet the Salient Characteristics in the Statement of Work (SOW). Vendor shall respond questions 1 through 8 of the Request of Information (RFI), see below. STATEMENT OF WORK Background: Bar Code Scanners are needed for the Phoenix VAMC ICU Ward to continue with Bar Code Medication Administration (BCMA) application. The procurement of the 25 scanners will assure continuity of bar code scanners at Phoenix VA HCS. All the scanners being compatible will facilitate continuity in the bar code scanner equipment, currently there are 3 generations of scanners within the ICU in both patient rooms and outside nurse workstations. The BCMA application is used to verify patient identity and match medication doses to the correct patient at the point of care. BCMA technology studies have proven to reduce medication errors. BCMA is a critical patient safety application that requires computers, scanner, and wristband printers to function properly. BCMA impacts nurses, pharmacists, providers, information technology staff and Veterans. Administration of medications requires scanning for the documentation of this procedure into the BCMA Application. This involves the recording medications administered and information pertinent to the care rendered. The Nursing staff is required to maintain a national standard minimum 93% scanning rate. This is to facilitate patient safety, the entry of information, ease of retrieval of records & for consistency in medication administration. Justification: Handheld bar-code scanners are required for the BCMA application and protocol throughout the facility and outpatient clinics. Who will be using the equipment being requested: Registered Nurses Respiratory Therapist Technical Information Enhanced Honeywell-based 1952HHD Cordless BCMA Firmware Scanner kit: Includes BCMA Firmware, setup card, FIPS 140-2, USB, 3-year Comprehensive warranty (TCG1902-1LC3) with TCG EWP Warranty Protection. Salient Characteristics VA Wireless Bar Code Scanner Requirements Ref # REQUIREMENT Mandatory Desireable 1 Scanner shall be compliant with FIPS 140-2 Y 2 Scanner shall not require additional external hardware to meet FIPS compliance Y 3 Scanner shall support a camera based area imager with a 2D imager engine bar code scanner Y 4 Scanner shall support one dimensional and two dimensional bar code capability Y 5 Scanner shall read all symbologies used for ISBT 128 blood bank standards Y 6 Scanner shall read all GS-1 standard bar code symbologies to include linear and GS-1 DataBar Y 7 Scanner shall be configurable to enable and disable the ability to read the composite portion of the GS-1 DataBar symbology Y 8 Scanner shall be pre-programmed to VA BCMA Defaults (scannable set-up ""cheat sheet"" is acceptable) (See Tab Below) Y 9 Scanner shall allow firmware upgrades to be installed in the field Y 10 Scanner firmware shall be backward and forward compatible Y 11 Scanner shall support omnidirectional scanning Y 12 Scanner shall support a minimum focal distance of not less than 3 inches Y 13 Scanner shall support scan area illumination providing visibility of the area being scanned Y 14 Scanner shall have an optional hands-free mode Y 15 Scanner shall support audible alerts that are adjustable by the end user Y 16 Scanner shall provide an audio or visual low battery alert/indicator Y 17 Scanner handset shall weigh less than 16 oz Y 18 Scanner shall have a transmission range of not less than 20 feet to base station Y 19 Scanner shall continue to function after a minimun 4 foot drop to concrete Y 20 Scanner shall have an IP seal rating greater than 40 Y 21 Scanner housing shall be constructed of or coated with health care grade plastic (antimicrobial and/or disinfectable with hospital grade cleaners) Y 22 Scanner base station shall connect using a standard USB interface Y 23 Base station shall use ""power over USB"" for charging scanner and communication (i.e. shall not require separate power source) Y 24 Scanner shall be charged without removing batteries Y 25 Scanner shall have a one piece pistol grip (i.e. pistol grip is not a separate piece/accessory) Y 26 Scanner retains settings after power interruption Y 27 Scanner battery life shall exceed 2000 recharge cycles Y 28 Scanner battery shall not develop charge memory Y 29 Scanner battery recharging time shall be less than 3 hours Y 30 Scanner shall not require tools to replace batteries Y 31 Warranty 3 years or more Y 32 Vendor provides 24/7 customer support during warranty period Y 33 Vendor/manufacturer provides customer notification of identified problems Y 34 Vendor provides firmware upgrades at no cost during warranty period Y Warranty 3-year Comprehensive warranty, with TCG EWP Warranty Protection. If warranty repair is done off-site, all hard drives or other data storage devices will be removed or purged of data, prior to release to contractor which bar code scanners. Security and HIPPA Compliance Considerations: In reference to VHA Handbook 6500.6 Appendix A, Blocks 6 and 7, the equipment included in this contract does not store and process ePHI over the hospital network.  Therefore C&A requirements apply, a security accreditation package is not required, and the following provisions from Appendix B & C apply. The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information protection, patient privacy, and information system security as delineated in this contract. During install, training, warranty repair and/or maintenance provided by the contractor, they may have physical administrative access to the equipment included in this contract that capture and process ePHI.  If warranty repair is done off-site, all hard drives or other data storage devices will be removed or purged of data, prior to release to contractor. Privacy and HIPAA Focused Training Web-Based (WBT): VA Course No. VA10203 is the official privacy training for VA employees, contractors, and volunteers who have access to protected health information (PHI) in the performance of their official duties or have access to VA systems (e.g. CPRS or VistA Web).  All contracted employees will complete this training prior to being granted access to the subject equipment, or to the data therein.  The training is available on the VA TMS website at: https://www.tms.va.gov.  Contractor will provide evidence of training to the COR, if requested. Any and all ePHI stored in the equipment will be transferred to either VistA Imaging or a dedicated server for long-term retention and incorporation into patient records. (Appendix B) The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information protection, patient privacy, and information system security as delineated in this contract. In reference to VHA Handbook 6500.6 Appendix A, Block 7, the equipment (or service) involves storing, generating, transmitting, or exchanging VA sensitive information. Therefore, the following from App C apply. . The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications.
