- The Great Lakes Acquisition Center (GLAC) in Milwaukee, WI is searching for vendors who have the capability to provide the below facilities software/hardware, media, software license, software maintenance/upgrade, installation, training, and technical support services for a Patient Checkout Discharge Instruction System. Software maintenance shall include periodic updates, enhancements and corrections to the software, new releases and documentation, and technical support. The Patient Checkout Discharge Instruction System shall interface with VA systems as well as provide the following: The software shall interface with the VA s Computerized Patient Record System (CPRS) (Electronic Medical Record) and Veterans Health Information Systems and Technology Architecture (VISTA). Electronic copy of the patient instructions shall be automatically sent to CPRS as a signed note in the CPRS NOTES tab. Patient check-in information shall be automatically populated to a patient selection list. Known primary care providers shall be automatically added for follow up referral list. Medications list and future appointments known in CPRS at the time of document creation shall be automatically included in the printed instructions for the patient. Base content shall be customized, or site-specific content can be added. Patient instructions shall be customizable by the document author at the time of document creation allowing for patient specific content. The patient specific content shall be printed and stored in the patient CPRS NOTE. Customized form templates shall be created by department/use case with configurable form menu options and reminders. Windows® Active Directory Authentication for improved password management. Auto creation of users using Windows® Active Directory integration. End users shall be able to select follow up care instructions based on follow up type. Health content library includes over 5,500 topics in English and Spanish written a 5th -6th grade reading level. Graphics included with some of the topics. Inpatient and Ambulatory Stage 1 and Stage 2 ONC-certified for Meaningful Use. Shall meet Joint Commission standards for patient and family education. All interested responsible sources shall submit a brief summary of their capabilities to this office by 10:00 a.m. Local Time on Friday, May 10th. The capabilities statement shall include a brief narrative on how their Patient Checkout Discharge Instruction System includes all the above requirements as well as how vendor will abide by the Limitations of Subcontracting rule, 852.219-75 (see clause below). These requests are mandatory for consideration. Please also provide the vendor s Unique Entity Identifier (UEI), experience with the Patient Checkout Discharge Instruction System mentioned above, and name and phone number of a point of contact in case further information is needed. This RFI is issued solely for information and planning purposes to determine availability of qualified vendors and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. The procurement will be competed under NACIS 513210.
