- The Navajo Area Indian Health Service, Gallup Service Unit in Gallup, New Mexico, intends to award a Blanket Purchase Agreement (BPA) with Myriad for various diagnostic and esoteric laboratory testing. The anticipated period of performance is date of award for one year. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 541380, Testing Laboratories and Services with a small business size standard of $16.5 million.� The Gallup Service Unit is also seeking capable Indian Owned Economic Enterprises (IEE) and Indian Owned Small Business Economic Enterprises (ISBEE) to various diagnostic and esoteric laboratory testing. Any IEE or ISBEE sources that believe they are capable of fulfilling the government requirements as described are encouraged to submit their capability statement, price lists, and catalogs to the Point of Contact listed below via email only and no later than 1:00 pm on May 1, 2024 (MDT). In accordance with the Buy Indian Act, 25 U.S.C. 47, the Indian Health Service shall give preference at all times, as far as practicable, to Indian economic enterprises.� If your firm is capable of providing the services described in this notice, complete the attached representation form and submit it along with the rest of the requested documents identified in this notice. AT A MINIMUM, THE FOLLOWING INFORMATION MUST BE SUBMITTED: Confirm the Buy-Indian set-aside status you qualify for under following NAICS Code: is�541380, Testing Laboratories and Services If the proposed NAICS is not the customary or applicable NAICS relative to the needs description, include your firm�s proposed NAICS and rationale for the different NAICS. If claiming IEE or ISBEE status, complete and submit the attached IHS Buy Indian Act Indian Economic Enterprise Representation Form. Provide firm�s UEI Number. Evidence of recent (within the last five years) experience with work similar in type and scope to include: Contract Numbers Project Titles Dollar Amounts Percent and complete description of work self-performed. In the event additional BPAs are deemed to be in the best interest of the government, capable vendors may be contacted.� This notice of intent is not a request for quotations and a solicitation will not be issued in conjunction with the notice of intent.
