- SECTION 1 VISN 5 Network Office Statement of Work (SOW) AudioCommunicator/ AudioCommunicator DM (Disease Management) Background: VA Capitol Healthcare Network (VISN 5) requires the addition of new AudioCare modules that add additional functionality and build continuity of use through to current AudioCare systems at each VISN 5 medical center. The Huntington VAMC currently has the two AudioCare modules in place and are included to have access to the annual license and support. Salient Characteristics Must interface with existing Audiocare system. Must be capable of notifying patients via phone calls. Must be able to automate run time and day of communications. Must be delivered with one base year and 4 additional option years. Deliverables: For Beckley, Clarksburg, and Washington DC VAMCs: -Install and ensure operation of AudioCare, AudioCommunicator standard application For Beckley, Clarksburg, Martinsburg, Baltimore, and Washington DC VAMCS: -Install and ensure operation of AudioCare, AudioCommunicator DM (Disease Management) application For Beckley, Huntington, Clarksburg, Martinsburg, Baltimore, and Washington DC VAMCS: -Annual license and support maintenance of AudioCare Communicator application -Annual license and support maintenance of AudioCare DM application Non-Compliance: The Contracting Officer or his/her designee will notify the contractor of any noncompliance with the foregoing provisions and the action to be taken. The Contractor shall, after receipt of such notice, immediately correct the conditions to which attention has been directed. Such notice, when served on the Contractor or his/her representative at the site of the work, shall be deemed sufficient for the purpose aforesaid. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all and any part of the work and hold the Contractor in default. Period of Performance The contractor shall deliver the product under this SOW within 90 calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor s proposal, the contractor s proposed completion date shall prevail. Work at the government site shall not take place on Federal holidays or weekends unless directed by the Contracting Officer (CO). Implementation The vendor will be responsible for configuring the system to the specification of the end users in VISN 5. Information housed in the current system must be migrated and available to VISN 5 once the upgraded system is implemented. This work must begin within 90 days of contractor award. The vendor will provide training to end users upon implementation of the upgraded system. Service/Annual Maintenance: VISN 5 requires service and annual maintenance for AudioCare AudioCommunictor and AudioCare AudioCommunicator DM systems. The vendor is to maintain optimal function of the upgraded systems. The vendor must provide the following support to VISN 5: Security Updates to Software Vendor to provide as available throughout the life of the contract. Updates and Upgrades to AudioCare Applications - Vendor will provide system updates and upgrades as needed to the application. Vendor to provide backup requirements and VA OIT will provide appropriate storage and backup as requested. Ad Hoc Troubleshooting - Upon request by VA staff who are reporting an issue with the AudioCare application, the vendor will perform troubleshooting efforts and to resolve all reported issues (e.g. review the log files, stopping/restarting services, etc.). Once an issue is reported, response time will be within 60 minutes of initial notification. VISN 5 operating hours are from 7am 5pm Monday-Friday EST. Application Modification Support: The vendor will support VISN 5 with all application modification that may be required to support operations and patient communication through the life of the contract. Annual License Renewal: All licenses (application-specific and any third-party licenses such as SQL) to operate the AudioCare application is to be included in this base plus 4 requirement to streamline acquisitions required to maintain functionality and operation of the system. VA can provide enterprise Microsoft operating and SQL licensing.
