Information required during presentation, unless marked as optional: 1. - Video of lab/lab capabilities (optional). 2. - Catalogue of tests and test interpretations vendor is capable of performing (attachment is acceptable). 3. - List of Turn Around Times for tests/interpretations in #2 (attachment is acceptable). 4. - Please explain your testing resources. How many laboratories do you own? How many of your testing locations are within the VISN19 footprint? What is the approximate total square feet of all vendor-owned labs? How many employees do you have that are qualified to run non-waived testing as deemed by CLIA? 5. How many tests and percentage of tests do you subcontract to other reference laboratories? For example, your reference lab can only perform glucose and sodium testing. Your lab receives a test request for glucose, sodium, and creatinine testing. Your lab can perform the first two tests but will have to send the creatinine to another reference lab for testing. You would answer this question as such: The number of tests we subcontract to other reference labs is one(1). The percentage of tests we send out to other reference labs is 33%. 6. - What accreditations do you hold? 7. - What departments do you offer? Example: Hematology 8. - How do you trace/track patient samples from hospital to result generated? 9. - How do notify the lab if there is a problem with a specimen? 10. - What courier services do you offer for the pickup and delivery of patient samples? Who is responsible for packaging and transporting patient samples? 11. - What is the average age of your analyzers used for Chemistry, Hematology, Coagulation, and Urinalysis (chemistry & sediment analysis) testing? 12. - Are you interfaceable with VA laboratory information systems, VistA and Cerner? 13. - Are there any industry advances, or changes in doing business, we should be aware of? (optional) 14. Please pick 5 of the following scenarios and explain the steps (who performs each step and how long each step normally takes) from hospital to test result (see example at bottom of list): BCR-ABL sample collected at Salt Lake City VA Medical Center (Salt Lake City, UT). Ceruloplasmin sample collected in a Blue Top tube (ie. incorrect sample type) from the Rocky Mountain Regional VA Medical Center (Aurora, CO). Free Testosterone sample collected from Fort Harrison VA Medical Center (Fort Harrison, MT). Confirmatory Tramadol test collected Friday evening at Lawton/Fort Sill VA Outpatient Clinic (Fort Sill, OK). A frozen stool sample for Helicobacter pylori antigen testing collected Wednesday at 4pm on 11/27/2024 (the day before Thanksgiving) at Cheyenne VA Medical Center (Cheyenne, WY). Haptoglobin sample collected at Grand Junction VA Medical Center (Grand Junction, CO). Lymphocyte Subset sample collected at Sheridan VA Medical Center on Sunday at 8am (Sheridan, WY). Urine sample collected at Billings Healthcare Center for Ethyl Glucuronide testing (Billings, MT). Frozen Rheumatoid Factor sample (incorrect storage temperature) collected at Jack C. Montgomery VA Medical Center (Muskogee, OK). Example of scenario #1 VA lab at SLC puts BCR-ABL sample in biohazard bag with manually written requisition slip. Out-of-this-World Reference Lab, Inc. has our courier pick up sample from hospital and drop off at reference lab located 30 minutes north. Sample is batched for next day testing. Results are faxed back to VA lab within 24 hours of receipt of sample in Out-of-this-World processing department.
