SAMPLE STATEMENT OF WORK SEE BELOW PERFORMANCE WORK STATEMENT (PWS) MIDMARK ELEVANCE DENTAL CHAIR INSTALL, REMOVAL, AND DISPOSAL OF EXISING AT VA MEDICAL CENTER RENO, NV General Information 1. GENERAL: This is a non-personnel services contract to provide 3 Midmark Elevance Dental Chairs, 3 Four position programmable foot switch chair controls, 3 J-Box Housings only, and 3 LED LR Operating lights with adapter mounting kits to replace the 3 current Engle 300 Dental Chairs, 3 Four position programmable foot switch chair controls, 3 junction box housings and 3 LED LR Operating lights with adapter mounting kits. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. Delivery Time of 3 Midmark Elevance Dental Chairs: 45 days ARO Installation Completion: 5 days after delivery of units 1.1 Scope: VA Medical Center in Reno, NV is currently seeking to purchase 3 new Midmark Elevance Dental chairs, 3 Four position programmable foot switch chair controls, 3 J-Box Housings and 3 LED LR operating lights with adapter kits, as well as to perform deinstall and dispose of the current 3 Engle 300 Dental Chairs, 3 Four position programmable foot switch chair controls , 3 junction box housings, 3 LED LR operating lights with mounting kits with installation services. The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform deinstall and dispose of the current 3 Engle 300 Dental Chairs, 3 Four position programmable foot switch chair controls, 3 junction box housings and 3 LED LR Operating lights with adapter mounting kits, as well as install the 3 new Midmark Elevance Ultraleather Plush Chairs, 3 Four position programmable foot switch chair controls, 3 J-Box Housings and 3 LED LR Operating lights with adapter mounting kits as defined in this Performance Work Statement except for those items specified as government furnished property and services. The contractor shall perform to the standards in this contract. Also includes with its Installation Services an operational check-out test, any necessary adjustments and a demonstration of the equipment for appropriate facility personnel. 1.2 Background: VA Reno Dental Service would like to replace the current 3 Engle 300 dental chairs, 3 Four position programmable foot switch chair controls, 3 junction box housings and 3 LED LR operating lights with adapter mounting kit that do not fully function and are at end-of-life. It is essential to have 3 reliable, functioning dental chairs, 3 Four programmable foot switch chair controls, 3 junction box housings, 3 LED LR operating lights with adapter mounting kits for efficient patient care and access, patient weight max of 450lbs. The 3 current Engle dental chairs, foot switch controls, junction boxes, and lights were installed in 2016, when the Dental clinic moved out of the main hospital into the current location of 3674 South Virginia St. Reno, Nv. The Dental clinic has 10 Midmark Elevance Dental chairs currently in use, these 3 replacement chairs will match to stay consistent within the clinic. This is a request for Brand Name Midmark Elevance only. Will not accept like or equal items. The contractor performing the install of Midmark Elevance brand names chairs must be an authorized distributor or reseller. The contractor must also be a certified installer of Midmark Elevance dental chairs. Contractor must provide certifications at the time of submission of their formal quote request. Refer to exhibits for photos of existing Engle dental chairs and space. Time frame for the install is 5 days after delivery. Personnel will determine the most efficient method of disposal including but not inclusive of pick up by carrier or removal by local scrapping entity. Please note, disposal of Customer equipment implies Customer relinquishes ownership rights of disposed equipment. Install of Midmark Elevance Dental Chairs and J-Box Housings, LED LR Operating Light and Light adapter kit Installation includes all necessary labor (non-union) and materials required to uncrate, set-in place and assemble Midmark Elevance Dental Chairs and J-Box Housings, LED LR Operating Light and Light adapter kit equipment. Providing a laydown area and disposal of packaging is the responsibility of the Customer. Seismic Anchoring (Excludes x-raying the floor, through bolting, & associated hardware) Provides a representative to anchor the equipment to ensure the seismic installation meets local building codes. (Excludes x-raying the floor, through bolting, & associated hardware) Where permitted, Seller will connect to Customer-supplied utilities within 2 feet of the final point of connection. Utilities shall not be separated from point of equipment connection by physical barriers such as walls, ceilings, or other impediments. Terms of Agreement: All work shall be completed during the business hours of Monday-Friday, 8am to 4pm, National Holidays excluded. Delivery, Install and Removal location: Dental Clinic 3674 S.
