- This notice is not a solicitation announcement and will not constitute any commitment by the Government. Neither unsolicited proposals nor other kinds of offers will be considered in response to this Sources Sought Notice. Market research is being conducted to gain knowledge of potential qualified small business sources to determine if they can effectively compete on a Firm Fixed Price�purchase order contract with the period of performance of (1) base year of 12-month and (3) 12-month option years. Medical Surveillance Mobile Units shall provide baseline and periodic occupational health examinations required for full-time technician employees and/or temporary technician employees who are or have the potential to become exposed to health hazards in the work environment. �Occupational Health and Medical Surveillance examination services will be provided to Federal employees of participating agencies in accordance with (IAW) the Occupational Safety and Health Administration (OSHA), Title 29 Code of Federal Regulations (CFR) � 1910, Army Regulation (AR) 40-501 Standards of Medical Fitness, National Guard Regulation (NGR) 40-5, The Army National Guard Preventive Medicine Program, AR 40-5 Preventive Medicine, and Department of Defense Instruction (DoDI) 6055.05-M). Exam requirements may include, but not limited to spirometry, audiogram, urinalysis, chest x-ray, vision screening, laboratory studies (CBC, CMP, Heavy Metals), medical history, physical examination, respiratory clearance, and fit testing. The mobile unit must be flexible to provide applicable services at three designated locations and accommodate a mass number of employees within an eight-hour period.� In addition, the company must provide a fixed facility, within 25 miles of Edgewood, Maryland, who offer medical surveillance services as requested. Mobile services will be provided at the following locations: Army Aviation Support Facility (AASF), Aberdeen Proving Ground, Bldg. E-4081, Edgewood, MD 21010 Camp Fretted Military Reservation (CFMR), Field Maintenance Shop (FMS) 1, 5525 Rue Saint Lo Dr., Reisterstown, MD� 21136 Havre De Grace Military Reservation (HDG), Combined Surface Maintenance Shop (CSMS), 301 Old Bay Lane, Havre De Grace, MD� 21708 This is a Market Research and Sources Sought Notice only. No award will result from this Sources Sought Notice. There is no solicitation available currently; therefore, do not request a copy of the solicitation. If a solicitation is released, it will be synopsized in accordance with the Federal Acquisition Regulation and its supplements. It is the contractor�s responsibility to monitor the Government Point of Entry for the release of any solicitation. The purpose of this sources sought notice is to determine the availability of qualified contractors to perform these services and to determine socioeconomic classifications (Small Business, HUBZone, Service-Disabled Veteran Owned Small Businesses, Women Owned Small Businesses, etc.) associated with each. The primary North American Industrial Classification System (NAICS) Code applicable to this requirement is�621498. Responses to this Sources Sought Synopsis will be used by the government in making appropriate acquisition decisions. RESPONSE INSTRUCTIONS Responses are requested with the following information: 1. Contractor�s name, address, points of contact with telephone numbers and e-mail addresses along with CAGE Code. 2. Business size/classification (to include any designations such as Small Business, HUBZone, Section 8(a) contractor, Service-Disabled Veteran owned Small Business, Small Disadvantaged Business, Women Owned Small Business, etc.) shall be indicated on the first page of submission. 3. Contractor shall identify the percentage of services provided to the commercial marketplace and the percentage of services provided to government agencies. 4. Documentation should be submitted with the following considerations: � � �a)� �Provide Offerors' name, address, points of contact with telephone numbers and e-mail addresses. Provide confirmation of active System for Award Management (SAM) registration. � � �b)� �Business size/classification to include any designations as Certified Small Business, HUBZone Certified Small Business Concern, Section 8(a) contractor, Service-disabled Veteran-owned Small Business, Small Disadvantaged Business, Women-Owned Small Business and Large Business Concern shall be indicated on first page of submission. ��� �c)� �Special Qualifications:� It is the responsibility of the contractor to ensure the health care team are current and appropriately licensed, trained, boarded and certified to perform medical surveillance screening and testing.� License and certifications will be made available upon request. They will operate under the supervision and direction of a BCOHP provided by the contractor. Experience in performance of similar contract tasks in the last five (5) years. No more than five (5) similar service contracts will be reviewed. Experience and ability to provide contract services that successfully demonstrate contractor's timeliness/effectiveness of contract problem resolution without extensive customer guidance. Experience and ability to incorporate and manage changes of requirements and/or priorities within scope of contract including planning, execution, and response to customer changes. Experience and ability to successfully respond to emergency and/or surge situations (including notifying Contracting Officer Representative or Contracting Officer in a timely manner regarding urgent contractual issues). �� �d) � Project information is not to exceed one (1) page per project and should include title, location, general description of the project, Offeror's role, dollar value of the project, and name of the company, agency, or Government entity for which the work was performed with contact information (reference name, phone number and email address). The above requested information shall not exceed a total of fifteen (15) pages on 8.5"" x 11"" paper, 12-point Times New Roman font, with a minimum of one (1) inch margins all around.� Please do not provide standard marketing brochures or catalogs. Responses will be evaluated based on demonstrated functional capability. 5. Responses are due no later than�Friday,�09 February 2024, 4:00 p.m. EST.
