- This Sources Sought Notice is for informational and planning purposes only and shall not be construed as a solicitation or as an obligation or commitment by the Government. This notice is intended strictly for Market Research. This is a Request for Information only. This is NOT a solicitation for proposals, proposal abstracts, or quotations. DISCLAIMER This RFI is issued only for information and planning purposes and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. IAW FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a sources sought synopsis only. This synopsis aims to gain knowledge of potential qualified sources and their size classification relative to NAICS 339112. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After a review of the responses to this source sought synopsis, a solicitation announcement may be published. Responses to this source sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 17 Network Contracting Office, is seeking sources that can provide Endoscope Reprocessor for the VA Valley Coastal Bend Health Care System. The information identified above is intended to be descriptive, not restrictive, and to indicate the quality of the supplies/services will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications. If you are interested and are capable of providing the sought-out supplies/services, please provide the requested information as well as the information indicated below. Response to this notice should include the company name, address, point of contact, and size of business pursuant to the following questions: (1) Is your business a: Service-Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), HUBZone, Woman Owned Small Business (WOSB), Large Business, etc.? (2) Is your company considered small under the NAICS code identified under this RFI? (3) Are you the manufacturer or a distributor of the items being researched above? (4) If you re a small business and you are an authorized distributor/reseller for the items identified above, do you alter; assemble; or modify; the items requested in any way? ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 2.00 EA Automate Endoscope Reprocessor/AER LOCAL STOCK NUMBER: 0002 2.00 EA Removal of old equipment 0003 2.00 EA Install of new equipment 0004 2.00 EA DISPOSAL OF AER General: Automated Endoscope Re-processor Description of Work: This statement of work defines the effort required for VA Texas Valley Coastal Bend Healthcare System (VATVCBHCS), Sterile Processing services to procure two Automated Endoscope Re-processors needed as they have reached the end of service life. Harlingen is the location requesting in the sterile processing department to facilitate the continuing demand for patient care in order to service veterans in a timely manner. Proposed high level disinfection equipment for the reprocessing of endoscopic devices and ultrasound probes. Place of Delivery - The vendor shall deliver the described herein to: VATVCBHCS VA Harlingen Healthcare Center (HCC) Receiving Dock 2601 Veterans Drive Harlingen, TX 78550 POC: Jaime Noyola, jaime.noyola@va.gov, 956-299-1861 Alt POC: Juan Muniz, juan.muniz3@va.gov, 956-291-9000, X69469 Period of Performance: Period of performance shall be done in a reasonable time frame (no longer than six months after receipt of order) unless unseen circumstances arise which need to be discussed with SPS and/or Biomed representative. Hours of Performance: Work shall occur Monday Friday during normal duty hours unless authorized by SPS. Work will be coordinated between VA POC and contractor during kick off meeting. Product Requirements for Automated Endoscope Re-processors (AER) 2 EACH Re-Processors (AER) must be able to provide high level disinfection for endoscopic devices (CF-HQ190L, PCF-H190DL, GIF-HQ190) and ultrasound probes (C10-3V). Compatibility with VCB owned GI scopes CF-HQ190L, PCF-H190DL, GIF-HQ190) and ultrasound probes (C10-3V) Contractor responsible for deinstallation, installation and removal of equipment to include all planning and support services in the execution of this replacement project. Training of staff in use of AER equipment. Biomed training for initial assessment and diagnostics of equipment must be available. AER must be compatible with existing piping ½ NPT, three tier filtration system, (1 micron, 0.4 micron, and 0.1 micron). AER must be compatible with JUN-AIR electric indoor air compressor. VA Texas Valley Coastal Bend Healthcare System (VATVCBHCS) open to existing Medivator Advantage (SN 66326330 and SN 66326336) trade in. Documentation: Contractor shall furnish a delivery manifest once all equipment has been delivered. Payment will not be processed until all equipment has been received by Sterile Processing Service. Payment will not be processed until a properly completed service report is received. The service report shall contain, at a minimum, the following information: Date and time of contractor s arrival on station Complete list of instruments and consumable delivered/required. The service report shall itemize every item in the specification. Each item shall state a complete description of all work performed concerning the items. Invoicing: Payment to be made by certified invoices and must contain the contract number in addition to the requirements detailed in 52.212-4 (G) to be considered valid. All invoices shall be submitted to the VA Financial Service Center and emailed to the CO. All invoices will reference the purchase order number assigned to the contract.
