Description
XPS Greenlight Laser Maintenance The Department of Veterans Affairs, Network Contracting Office 7 located in Columbia, SC intends to issue a sole source firm fixed priced contract to include a three hundred and sixty five day (365) base period and four additional three hundred and sixty-five (365) day option years, for a total of five (5) years, to Boston Scientific Corporation, 300 Boston Scientific Way, Marlboro, MA 01752. The Boston Scientific XPS Greenlight Laser equipment is proprietary to Boston Scientific, a large business manufacturer; no opportunities exist for any Service Disabled, Veteran Owned Business or for any other resellers, as only Boston Scientific can perform maintenance, including and not limited to software updates without any loss of existing warranty coverage for this equipment. Interested parties may furnish information by email only with Response to Intent to Sole Source for the XPS Greenlight Laser Maintenance within the subject line. All documents submitted will not be returned. All interested parties shall submit clear and convincing documentation demonstrating their capabilities to satisfy the requirements listed to Joseph Locke by email at joseph.locke@va.gov. The capability documentation should include general information and technical background describing in detail the prospective contractor's capabilities and experience that will demonstrate the ability to support this requirement. GENERAL INFORMATION Title of Project: Full-Service Contract - GreenLight XPS Laser Therapy System Scope: Contractor shall provide full corrective and preventative maintenance services as outlined below. Location: 109 Bee St, Charleston, SC 29401 Performance Period: Monday Friday, 8:00 a.m. to 5:00 p.m. excluding federal holidays. Type of Contract: Firm-Fixed Price SCOPE The contactor will provide corrective and preventative maintenance on the Greenlight XPS Laser System (SN: XPS50743) at RHJVAMC. The GreenLight XPS Laser Therapy System is located at 109 Bee Street, Charleston, South Carolina. Required Service Two preventative maintenance inspections every 12-months, i.e., semi-annually 100% coverage on parts, travel, and labor charges This includes the resonator, laser source, optical components, electrical, and cooling components. 100% coverage on replacement console/parts Contractor will provide a free loaner for the duration of repair. All system software updates are included in this contract. Includes 24/7 phone support. Includes 48 hour response time. Contractor must be certified by the OEM (Boston Scientific) to work on the equipment. A copy of the training certificate from Boston Scientific is required to prove contractor qualifications. CHANGES TO SCOPE OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. TRAVEL All work is to be conducted at the Ralph H. Johnson VAMC located at 109 Bee Street, Charleston, South Carolina. The contractor is responsible for any anticipated travel and per diem. REPORTING: Vendor shall report to and sign-in at the RHJVAMC HTM office and obtain an Identification badge which shall be always worn while on station. After all work is completed, the Vendor must again report in person to the RHJVAMC. HTM office, to submit in writing, a complete report of services or repairs performed for each item of equipment. An electronic report of work performed (PDF, MS Word, etc.) shall also be submitted via email within 24-72 hours of each service visit. NOTE: Payment of invoices may be delayed if the appropriate reports are not properly completed and submitted to the RHJVAMC HTM Service as required above. HOURS OF WORK: RHJVAMC normal working hours are Monday through Friday 0800-1700 (with 30 mins for lunch), excluding federal holidays. All service shall be performed during normal working hours, unless the Contracting Officer s Representative (COR) provides written consent to the Vendor. The facility observes the following federal holidays: New Year s Day Birthday of Martin Luther King, Jr. Washington s Birthday Memorial Day Juneteenth National Independence Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Day Christmas Day RESPONSE TIME: The Vendor s on-site response time for service calls shall be two (2) business days (typically within 48 hours). TEST EQUIPMENT: RHJVAMC will not furnish test equipment for the performance of this contract. It is the responsibility of the Contractor to bring the appropriate equipment and/or supplies necessary to complete the work as required within unless otherwise specified elsewhere in this document. SPECIAL CONTRACT REQUIREMENTS: Vendor must follow all Department of Veterans Affairs regulations when working on VA equipment. Vendor shall be escorted by RHJVAMC Biomedical Engineering staff while working in sensitive areas of the facility (i.e. patient care areas, etc.). TYPE OF CONTRACT RHJVAMC anticipates award of a FIRM FIXED PRICE contract based on the contents of this solicitation.
