- Last Updated: 02/01/22 Page 1 of 1 DEPARTMENT OF VETERANS AFFAIRS Justification for Single Source Awards IAW FAR 13.106-1 For Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K) Acquisition Plan Action ID: 36C252-24-AP-0828 Contracting Activity: Department of Veterans Affairs, VISN 12, Edward Hines Jr. and Clement J. Zablocki Medical Centers, Great Lakes Acquisitions Center, NCO 12. 2237s: 578-24-2-3947-0011 695-24-2-3947-0017 Brief Description of Supplies/ Services required and the intended use/Estimated Amount: VISN 12 Pathology and Laboratory Medicine Services (PLMS) is requesting approval for a new Firm Fixed-Price Base Plus one year option year contract with Sebia, Inc. for automated electrophoresis and immunofixation cost per test (CPT). The cost per test shall include equipment, reagents, consumable supplies, preventative maintenance, and emergency repair service. The total estimated price for this action is approximately $240,460.00. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Due to the proprietary technology, Sebia, Inc. is the sole manufacturer and the only authorized distributor of the equipment and reagents currently in use at Edward Hines, Jr. VA Hospital and Clement J. Zablocki VA Medical Center. Since the VISN already owns Sebia, Inc s equipment and their requirements only consist of the tests/reagents/consumables, and maintenance no other company s tests/reagents/consumables is compatible in their currently owned equipment. Not procuring from Sebia would require purchasing all new equipment which would be cost prohibitive causing VISN 12 to send testing to a commercial reference laboratory. Additionally, this would delay patient care. Description of market research conducted and results or statement why it was not conducted: Market research conducted on VIP yielded zero possible SDVOSBs or VOSBs. Searching DSBS provided one company likely expected to provide the required items (Sebia, Inc). In conclusion, Sebia, Inc is the only one responsible source expected to provide the required items for a fair and reasonable price. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b).
