Description
AMENDMENT TO PRESOLICITATION NOTICE: This notice is an amendment to Pre-Solicitation Notice 36C26224Q0055 and the associated Request for Quote (RFQ) for Wheelchair and Car Transportation with Ad Hoc Medical Companion Services. 36C26224Q0055 was posted on 12/6/2023 and quotes were originally due on 12/20/2023 at 10:00 AM Mountain Standard Time. The response deadline to this RFQ is now extended to 12/21/2023 at 10:00 AM Mountain Standard Time. The following pages are in response to interested vendor questions accompanied by a separately attached revised Statement of Work. Responses are due by 10:00 Mountain Standard Time December 21, 2023. Point of contact information: Lori Gammage, Contract Specialist, Lorenza.gammage@va.gov. Amendment to the Solicitation: This amendment incorporates the following changes to the RFQ: 1. Refer to E 3 52.212-2 Evaluation Commercial Products and Services (Nov 2021) Page 47 under Technical, number 3 3. The quoter will be evaluated on the evidence that they provided listing the transportation assets that will be used in support of the contract. This list will include number of vehicles, location, and specifications of wheeled and hired cars that will be used in the performance of this contract. Updated to: 3. The quoter shall submit evidence they have the capability to provide vehicles that can support wheelchair and hired car transportation in accordance with the Statement of Work. 2. Refer to D1. Statement of Work Page 26, 16 Inspection and Maintenance 16. The VA San Diego Healthcare System reserves the right to inspect the establishment, facilities, equipment, vehicles, business reputation, and qualifications of the successful offeror, prior to award and in accordance with the Pre-Award Survey Checklist. Contracting Officer shall coordinate the meeting with the contractor and VA staff that will be attending the site visit to conduct the Pre-Award Survey, to determine if the contractor is lacking any of the items and/or equipment listed on the pre-award survey checklist. This is to ensure acceptable standards and requirements of the contract. If any vehicles are added after inspection and after award, the contracting officer, or his designated representative at the VA Medical Center, reserves the right to inspect and approve the vehicles before being placed under the contract. Complete maintenance records must be maintained by the contractor and available to the COR during contract performance. Updated to: 16. Answers to questions submitted by interested vendors are provided below: Question 1. On the price/cost schedule in section B.2, that items 0007 and 0008 are the same description. Is this an error by chance. Could you please review and confirm whether this is a mistake, or correct? VA Response: CLIN 0008 was a typo. CLIN 0007 is correct. Corrected as listed below to read: Current: CLIN 0008 Hired Car Billable Incomplete Pickups Qty 240 each Updated to: CLIN 0008 Waiting Time Qty 240 each is Correct.
