Request for Information (RFI) DB - Install Fall Protection Anchorage and Guardrails for Bldg. 1 and 2 PROJECT # 673-23-104/ 36C24824R0016 Attachment 2 RFI Tracking Log RFI NBR Question/ Response Company Submitted By Dated Received 1. First to define the Fall Protection world differences: Fall Prevention System: Prevents people from reaching the fall hazard by creating a barrier between the person and the fall hazard, such as passive guardrails. Fall Restraint System: Prevents people from reaching a fall hazard through a tie off system such a horizontal lifeline (e.g., a cable traveler system) Fall Arrest System: Stops a fall that is in progress through a tie off system. Suspended Access System: Also considered rope access anchors, window cleaning anchors, facade maintenance anchors, or tie-backs. This is an anchor system that allows for suspending a worker to conduct facade maintenance work such as window cleaning. A suspended access system can double as a fall restraint or fall prevention system. So typically, you would not need to add fall prevention guardrails where suspended access anchors are newly installed. Please confirm which of the above is desired for the following locations: Bldg. 1 Penthouse roof, Bldg. 1 3rd floor roof,  bldg. 2 roof and bldg. 2 penthouse roof. Midwestern 11/22/23 VA Response: : Bldg. 1 Penthouse roof: Guardrails Bldg. 1 3rd floor roof: Guardrails and anchors Bldg. 2 roof: Guardrails and anchors Bldg. 2 penthouse roof: Guardrails Please review the last pages and the pictures attached for further clarification of areas lower than 3rd floor roof 2. For Building 1, there are some areas of the roof that do not have existing fall prevention guardrails. In these areas, will the installation of suspended access anchors suffice or are you wanting to add guardrails anyway? Midwestern 11/22/23 VA Response: The guardrails that are existing should remain. The new guardrails will fill in the areas that are not currently covered. The project is providing safety measures for 2 sets of people. The safety of untrained persons and trained persons. Untrained persons may not know how to use fall protection and the guard rails will be installed for their safety. The suspended access anchors will be used by contractors and equipment for maintaining the buildings. 3. For Building 1, the upper large roof has guardrails already installed. In these areas, will the installation of suspended access anchors be the only thing required? Midwestern 11/22/23 VA Response: Correct 4. For Building 1, do you want suspended access anchors to reach every facade face of the building, even if there are no windows? Midwestern 11/22/23 VA Response: Correct, the anchors can be used for other maintenance activities. 5 For Building 2, there appears to be no fall protection of any sort. Do you want suspended access anchors to reach every facade face of the building, even if there are no windows? Midwestern 11/22/23 VA Response: Correct. The east façade side would not need to have them. It doesn t look like a suspended scaffold could be used there. 6 For Building 2, if a suspended access system is installed and can double as fall restraint or fall prevention, do you want to install guardrails? Midwestern 11/22/23 VA Response: Yes 7 Please clarify if a superintendent can be a dual hat to cover the requirements of SSHO? Blue Cord 11/22/23 VA Response: No 8 If no to the question above, will the SSHO need to be full-time for this project? Blue Cord 11/22/23 VA Response: Yes, due to the type of project this is, the SSHO will have to be onsite and active in the installation process. 9 Please clarify if a superintendent can be a dual hat to cover the requirement of QA/QC? Blue Cord 11/22/23 VA Response: Yes 10 If no to the question above, will the QA/QC need to be full time for this project Blue Cord 11/22/23 VA Response: N/A 11 Can a superintendent be a triple hat to cover the requirements of SSHO and QA/QC? Blue Cord 11/22/23 VA Response: No 12 Please provide architectural drawings for all roofs that are included in this project. Blue Cord 11/22/23 VA Response: Attached 13 Please provide structural drawings for Building 2. Blue Cord 11/22/23 VA Response: Attached 14 Please confirm that that any existing railing is to remain in place and worked around Blue Cord 11/22/23 VA Response: Confirmed 15 Please confirm if there are any warranties requirements for the roofs as part of this project Blue Cord 11/22/23 VA Response: There are no specific warranty requirements (that apply to the existing roof system) identified. The contractor should be prepared to research the roof material and review warranty with any manufacturer s requirements. This should be a part of the design phase. 16 If the answer to the above question is yes, please provide the warranty requirements for these roof systems. Blue Cord 11/22/23 VA Response: Attached 17 Please confirm that the roof above the new Emergency Department is not included in this project. Blue Cord 11/22/23 VA Response: It has been decided that it is included, please review pictures at the end of this document 18 Please confirm that interventional radiology suite is not included in this project. Blue Cord 11/22/23 VA Response: It has been decided it is included, please review pictures at the end of this document 19 Please confirm that the upper penthouse roof of Building 1 is only receiving guard rail. Blue Cord 11/22/23 VA Response: Confirmed 20 Please confirm that the south sub roof of Building 1 is included in this project. Blue Cord 11/22/23 VA Response: It has been decided that it is included, please review pictures at the end of this document 21 Please provide the existing master label for the campus lightning protection system Blue Cord 11/22/23 VA Response: Unable to locate this label 22 If one does not exist, will the contractor need to provide a new master label lightning protection system for the entire campus or will the contractor only need to provide certification for the new work associated with this project Blue Cord 11/22/23 VA Response: New work associated only 23 Are we required to fix any issues with the existing lighting protection, railing and roofing systems as part of this project? Blue Cord 11/22/23 VA Response: No 24 Please provide a hazardous material survey for all areas of this project Blue Cord 11/22/23 VA Response: We will provide AHERA for review, but a NESHAP is required prior to beginning work in all areas the contractor expects to work in 25 Past Performance: If we have a PPQ that is less than 12 months old, can we use it within our proposal? Or will a new PPQ need to be requested? Blue Cord 11/22/23 VA Response: That will be acceptable 27 Please confirm it is the VA s intent to use a horizontal lifeline system and not individual fall protection anchors Blue Cord 11/22/23 VA Response: That is correct 28 Does the VA have a preferred basis of design for the horizontal lifeline system? Blue Cord 11/22/23 VA Response: The safety anchor system design shall comply with current OSHA, ANSI, and local regulations pertaining to window cleaning and fall protection. 29 Please confirm which rooftop locations received fall protection and guardrails. Some areas were observed at the site walk with existing railing or fall protection systems. Blue Cord 11/22/23 VA Response: Bldg. 1 Penthouse roof: Guardrails Bldg. 1 3rd floor roof: Guardrails and anchors Bldg. 2 roof: Guardrails and anchors Bldg. 2 penthouse roof: Guardrails Please review pictures at the end of this document 30 During the site walk several existing railing systems were observed. Is it the intent of the VA for these to be removed, the roof patched, and a new railing installed? Blue Cord 11/22/23 VA Response: The intent is to leave the existing and install new to fill in the spaces that are not protected with guardrail 31 During the site walk several temporary roof patches and sandbags were observed. Please confirm that the repair of existing roof leaks is excluded from the scope of this project. Blue Cord 11/22/23 VA Response: This project is not responsible for any roof repair outside of the area affected by the new work installed 32 Please confirm the VA s intended finish for the new railings (I.e. Aluminum, galvanized, painted) Blue Cord 11/22/23 VA Response: Aluminum would be preferred. There is an aesthetic piece to this project and the product should match the majority of existing. 33 Please provide a section view of the existing roof and parapet structure. Blue Cord 11/22/23 VA Response: Attached 34 Does the VA have a preferred basis of design for the suspended scaffolding anchors? Blue Cord 11/22/23 VA Response: No 35 Does the VA have a preferred basis of design for the tie back anchors? Blue Cord 11/22/23 VA Response: It should follow the rest of the existing campus design 36 Does the VA have a preference on the spacing of suspended scaffolding anchors? Blue Cord 11/22/23 VA Response: No 37 Is the contractor required to provide the entire suspended scaffolding system, or just the anchors? Blue Cord 11/22/23 VA Response: Only the anchors 38 Some walls may be impossible to cover with suspended scaffolding due to obstructions on the roof. Is it the intent of the VA to have every wall set up to use suspended scaffolding and tie back anchors? Blue Cord 11/22/23 VA Response: Yes, every façade that can benefit from tuck pointing and window cleaning etc. 39 If no to the question above, which walls are required to be covered by suspended scaffolding? Blue Cord 11/22/23 VA Response: NA 40 Has the VA investigated whether the existing structure can support suspended scaffolding anchors, or if structural reinforcements will need to be made? Blue Cord 11/22/23 VA Response: No structural survey has been completed. 41 If the VA requires swing stage scaffolding, this may require the roof structure to be reinforced from underneath. How much of the 2nd Floor of Building 1, 7th Floor of Building 1 and 2nd Floor of Building 2 be made available to the contractor at one time Blue Cord 11/22/23 VA Response: There are many variables affecting a definite answer for this RFI question. The COR will work with the affected departments to accommodate any underside work. Please understand that these areas include medical and surgical patient areas. Depending on the contractor solution for the anchors, the underside work may require Infection Control measures as well. This installation will have to incorporate a plan to complete sections for this coordination. 42 Please provide a hazardous material survey for each area underneath all roofs affected by this project. Blue Cord 11/22/23 VA Response: The AHERA will be provided. All areas affected will require a NESHAP from the contractor to identify hazmat. 43 Please advise if in Factor II, part 4a if the year is intended to say 1999 or if another year should be referenced?
