- NCO 5 is seeking responses from businesses that are interested in this procurement and consider themselves to have the resources and capabilities necessary to provide these services. Please respond with your: 1. Business name, SAM UEI, business type, socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.), and person of contact (including telephone number & email address). 2. GSA contract number, if applicable, including the contract s ultimate completion date inclusive of all remaining option periods. 3. Capability statement and summary of experience conducting these or similar services for VHA and other entities. 4. Approximate cost estimate to perform the work described. Likewise, you are encouraged to suggest a pricing structure that could be used in the future contract (e.g., unit price per task, equipment, setup, etc.). Responses must be submitted by 13:00 (EST) November 16, 2023. Responses to the information requested above may be submitted via email ONLY to Kenneth.mcneill@va.gov. This request for information/sources sought notice is for planning purposes ONLY. The results of this market research will assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., small business set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted as a result of this notice. STATEMENT OF WORK General Intention: It is the general intention of the Louis A. Johnson VAMC located at 1 Medical Drive Clarksburg WV 26301 to repair the Cat/Skid steer, for the purpose of winter snow removal of loading the salt trucks, maintaining the grounds at the Louis A. Johnson VAMC. Contractor Requirements: Contractor shall provide the repairs to the Cat/Skid steer at the price quoted providing all parts and labor and to provide a warranty on all parts and labor under this repair. Contractor will provide the transport of Cat/Skid Steer to their facilities for repairs and return to the Louis A. Johnson VA Med Center when repairs are complete. Contractor will supply all parts and labor for repairs on Cat/Skid Steer. Contractor will make sure the Cat/Skid Steer is in proper operating order and all obligations are fulfilled set forth in the quote. Service Description: Replace Fuel Hoses to and from Fuel Filter Replace Bolts Replace both Tilt Cylinders Complete Bucket Mounting Plate w/Hardware, Latch Cylinders, & Pivot Pins All Hoses for Tilt Cylinders and Bucket Release Cylinders Replace Bucket Cutting Edge Replace Throttle Foot Pedal Cab Door Glass Spring Cylinder and Ball Socket
